Responsibilities:
Planning
Assesses the needs of individual residents and organizes for the provisions of resident care.
Assesses and documents MDS RAI within the required timelines.
Identifies both priorities and the appropriate nursing interventions.
Contributes input into establishing and implementing the policies and procedures for the nursing department.
Understands that performance will be evaluated based on compliance with Occupational Health and Safety Act Section 28 and Policy 2.1

MDS-RAI Coding
Providing RAI-MDS education and software training (Point Click Care) to members of the home and interdisciplinary team. Assist and support staff with access to AIS Mastering RAI MDS and Mandatory Competency Evaluation.
Monitoring and auditing completion of MDS assessments to ensure assessments are completed on time and that all MDS timelines are met and adhered to.
Assisting MDS users in coding assessments - providing materials, assistance and support to ensure coding accuracy.
Completing MDS data audits to confirm accuracy of coding on a quarterly basis.
Generating RAI-MDS output reports, QI and reviews with the management team and front-line staff, facilitating their use in clinical and operational decision-making and quality improvement.
Ensuring MDS assessments, resident assessment protocols and care planning are completed within mandated timelines.
Works with (RN/RPN) and Restorative nursing assistants on Nursing Restorative assessments, evaluations and discharges.
Works with RN/RPN on Wound & Skin Care Management
Continuous Quality Improvement
Contributes to the resident plan of care (both development and modification) through observation thus providing a constant update of the resident.
Ensures the maximum safety of residents and staff.
Performs competently within the Standards of Practice as outlined by the College of Nurses of Ontario.
Maintains the confidentiality of all information including material from and about the residents, tenants, families and colleagues.
Communicates effectively in both written and oral communications in compliance with expectations of current legislation.
Demonstrates a commitment to the philosophy and objectives of the nursing department of The Wexford.
Maintains competence relative to current practices.
Provides safe medication administration to residents.
Under the supervision of a Registered Nurse, maintains the safe keeping of all drugs and prescribed treatments on the unit
Functions within the policies and practices of The Wexford.
Completes documentation for monthly statistical data.
Successfully completes AIS Proficiency Evaluation annually.
Completes all assigned mandatory education annually
Leadership
Seeks help and guidance when unable to perform competently.
Assumes accountability for own actions.
Responsible and accountable for activities delegated by the Director of Care/Nurse Manager/Registered Nurse.
Maintains a good attendance record.
Follows good habits of punctuality, neatness and appropriate attire.

Participates in all aspects of the nursing process.
Implements the resident plan of care.
Completes assignments within scheduled time frames.
Qualifications and Competencies
Registered Practical Nurse with a current certificate of competence with the College of Nurses of Ontario
Experience in Long-Term Care
Must hold a medication administration certificate from a recognized Community College since 1985
A valid Vulnerable Sector Check for Employment completed by your local Police Department - No older than 6 months.
Two-step TB test or chest x-ray - No older than 6 months this has to be fill out within 14 days of hire*.
