The Transitional Care Unit is a 40 bed unit that cares for patients who have a discharge destination but face a temporary obstacle, such as awaiting home and community care services, rehabilitation and convalescent care, Hospice or retirement home/Assisted living. Focused, short term supports will enable these patients to be more quickly and safely discharged. Patients are transferred to this unit once a discharge plan is in place. The healthcare team consists of physicians, registered nurses, registered practical nurses, patient care assistants, dietitians, pharmacists, physiotherapists, occupational therapists, a discharge planner, social worker, a respiratory therapist, a speech language pathologist, Recreational Therapist,, unit clerks and volunteers. Each member has a specific role to play to ensure optimal health and seamless transitions. They strive to provide exceptional care to each patient and family. As the Nursing Unit Manager you are responsible to oversee the delivery of patient services in his/her department including safe, quality patient care and efficient patient flow. The Nursing Unit Manager prepares budgets and manages department resources. The Nursing Unit Manager develops a human resource plan for the department and provides effective leadership for the people in the department. The Nursing Unit Manager also contributes to Program planning and operations, as well as participating in hospital-wide initiatives. As a qualified candidate you will have the following job requirements:

  • Current Certificate of Competence from the College of Nurses of Ontario or Current registration with other applicable regulated health professional College of Ontario
  • Baccalaureate preparation in a health related field essential
  • Master's preparation in a health related field preferred
  • Three to five years current progressive experience in an acute care setting
Extensive knowledge and experience with Alternative Level of Care and Rehabilitation
  • Previous management experience an asset
  • Working knowledge of computer, Microsoft Office and Outlook applications
  • Working knowledge and understanding of Patient Information systems (i.e. Meditech)
  • Working knowledge and understanding of Financial Management systems
  • Working knowledge and understanding of Workload Measurement Systems (i.e. GRASP)
  • Financial Management - Education and training
  • Leadership training
