We are seeking a compassionate and detail-oriented
Licensed Practical Nurse (LPN)
to perform
home care assessments
for clients in their residences. The LPN will be responsible for conducting in-home evaluations, identifying care needs, and supporting the development of personalized care plans in collaboration with the healthcare team. This role is essential to ensuring our clients receive safe, appropriate, and client-centered care in the comfort of their homes.
Key Responsibilities:
Conduct initial and follow-up
home care assessments
for clients to determine physical, emotional, and environmental needs.
Document client status, health conditions, medications, and support systems using standardized assessment tools.
Collaborate with RNs, care coordinators, physicians, and allied health professionals to develop and update individualized care plans.
Monitor client progress and communicate any changes in health status or environment that may impact care delivery.
Educate clients and family members on care procedures, medication management, safety, and healthy lifestyle practices.
Ensure compliance with agency policies, Ministry of Health regulations, and professional nursing standards.
Maintain accurate and timely documentation in the client's health records and electronic systems.
Participate in interdisciplinary case conferences and contribute to quality improvement initiatives.
Qualifications:
Current registration and in good standing with the [provincial] College of Nurses as a
Licensed Practical Nurse (LPN)
.
Minimum 1-2 years of nursing experience, preferably in home care, community health, or geriatric care.
Strong assessment and critical thinking skills.
Excellent verbal and written communication skills.
Ability to work independently in a community setting and manage a flexible schedule.
Valid driver's license and reliable vehicle (required).
CPR/First Aid Certification (required).
Proficiency with electronic health record systems is an asset.
Work Environment:
Travel required between client homes within [service area].
PPE and safety protocols followed in all client visits.
