Job Summary

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care in various settings, including nursing homes, hospitals, and home care environments. As an LPN, you will work under the supervision of registered nurses and physicians to deliver essential medical services and support to patients of all ages.

Duties

Administer medications and treatments as prescribed by healthcare providers. Monitor patient vital signs and report any changes to the supervising nurse or physician. Assist with daily living activities, including bathing, dressing, and feeding patients. Provide care for patients with specific needs such as dementia, pediatrics, or hospice care. Maintain accurate patient records using electronic health record systems like Epic or Cerner. Perform phlebotomy and laboratory tests as required. Educate patients and their families about health conditions and provide guidance on care plans. Collaborate with healthcare team members to develop and implement patient care plans. Ensure compliance with medical terminology and documentation standards. Participate in case management activities to coordinate patient care effectively.

Qualifications

Valid LPN license in the state of practice. Previous nursing home, hospital, or acute care experience preferred. Experience in dementia care, pediatrics, home care, senior care, or hospice care is a plus. Strong knowledge of medication administration and pain management techniques. Familiarity with medical office procedures and terminology is beneficial. Excellent communication skills and the ability to work collaboratively within a healthcare team.
