Nursing Home Without Walls

LPN Coordinator

:

-Support the development of and execute community outreach programs for the older population to meet the objectives of the program.

-Ensure seniors have access to appropriate services and information relating to aging in place.

-Provide social health initiatives to counter social isolation and loneliness to seniors and their care givers.

-Increase knowledge on health-related issues important to aging in place and healthy aging for seniors and their caregivers.

-Empower the local community to respond to the needs of an aging population.

-Oversee and manage the Plaster Rock Nursing Home Without Walls team.

-Develop programs that address social isolation.

-Provide health promotion opportunities.

-Provide a supportive community environment for seniors and their caregivers.

-support interventions for aging in place.

-Build capacity of social contacts so that vulnerable seniors can safely remain at home.

-Provide opportunities for members of the community to collaborate with the team to support seniors in their community.

-Develop any other relevant initiatives that may arise during the program.

Qualifications:

-

College or University education in such areas as health care professionals, and/or health and well-being studies.

-current and active LPN license

-excellent communication skills both verbal and written and the ability to build positive relations and collaborations.

-knowledge of aging, age-friendly communities and resources as well as issues relating to the older population.

-an understanding of performance measurement and evaluation.

-an understanding of project management

-excellent self-motivation and problem-solving skills.

-ability to present information to individuals and groups.

-experience in engaging people through assessments, and/or group work.

Monday to Friday 8am-4pm. May involve the occasional evening and weekend as required, based on the needs of the clients.

Job Type: Full-time

Pay: $81,000.00-$82,000.00 per year
