Link to Treatment (L2T) Care Coordinator
to join our team in helping individuals who are newly diagnosed with HIV/HCV access care and treatment. This role is responsible for coordinating L2T service with other care services and supporting individuals in maintaining health within a harm reduction philosophy.
Key Responsibilities
Conducting admission and discharge of individuals to the L2T program
Promoting community connection and link to social support, relevant health, income and social services
Developing and monitoring care plans and comprehensive strategies to meet treatment and care needs by coordinating services
Assisting patients with their mental health and addiction concerns, including coordinating plans of care with Island Health teams
Coordinating access to housing through BC Housing or local housing providers and advocates for rent subsidies or grants
Supporting medication management by providing monitoring strategies, referrals to support groups, and facilitating access to viral load testing
Required Qualifications
BSN, BSW with current affiliation with a professional body, or equivalent degree with related experience. MSW or related graduate degree, plus related experience of 3-5 years is preferred.
Knowledge and experience working with priority populations with HIV/HCV within a case management environment
Experience working with, and extensive knowledge of, mental health and substance use services
Excellent oral and written communication skills
Well-developed planning, organization, and administrative skills
Maintains current knowledge of best practices for HIV/HCV treatment and adherence
Able to communicate and implement approved harm reduction strategies in meaningful ways
Maintains mastery level understanding of harm reduction, HIV/HCV, addiction, mental health, and the impact of poverty on health
Awareness of local services and supports to facilitate referrals as needed
Professional, collaborative, and team-oriented
First Aid & CPR certification
Non-Violent Crisis Intervention or Mandt
Must have access to a vehicle, hold a valid B.C. Class 7 or above, and have business insurance with $3,000,000 liability
Must be able to pass a CRRP Criminal Record Check
Experience facilitating groups is an asset
