Position Summary




Are you a LifeSkills Support Worker looking to highlight your knowledge, skills, and abilities in a new and dynamic environment? We currently have an opportunity for you to bring your excellent work ethic to our Primary Care department.



In accordance with Northern Health's vision of an idealized system of services built on a foundation of Primary Care where people and their families receive primary care services in Primary Care Homes supported by interprofessional teams the Primary Care Life Skills Support Worker functions as a member of the interprofessional team, under the direction of the Team Leader. This position applies best practice to provide people with services as outlined in an individual's plan of care, with the goal of maximizing independent living.



Shift Rotation/Hours of work:

Monday to Friday, Days - 08:30 to 16:30



Primary and Community Care

The quality of care increases by providing continuity of care over time and across settings and will result in better long-term health outcomes.

The Chetwynd Primary Care Clinic

boasts over 6,000 square feet of nature-inspired design harmonizing with the facility's natural surroundings. It contains 12 exam rooms, office space, 1 procedure CPR room with AED. Services provided by physicians, nurse practitioners, nurses and support staff in the clinic include:

Public health nursing Home and community nursing Home Support Aboriginal Liaison Mental Health and Addictions

In addition, specialized services are offered in the clinic on a weekly

basis, including:

Dental Dietitian Audio


Chetwynd

lies in the foothills of the Eastern slope of the Rocky Mountains and is the natural transportation hub of the Peace River Country. The area is rich in oil, gas and timber and the local resources and manufacturing industries reflect these assets. Other major industries in the area are ranching, tourism, mining and the service sector. The population of Chetwynd is 3,100, with a trading area of 7,000. There are three elementary schools, one Montessori school, one secondary school and a Northern Lights College campus. Qualifications




Diploma from a recognized Social Services Support Worker educational program, or Residential Care Aide program, two (2) years' recent, related experience working with people with complex health issues, or an equivalent combination of education, training and experience Or Grade 12 plus a Community Mental Health Worker Certificate and Addictions Studies Certificate plus three (3) years recent related experience working with adults with mental illness and/or addictions diagnosis; or an equivalent combination of education, training and experience. Valid B.C. driver's license. Valid First Aid Certificate, CPR and Food Safe.
