Role Overview
As a Licensed Practical Nurse (LPN), your leadership and priorities set the tone for the Community's environment. Using a collaborative approach with Residents and their entire Care Team including the Resident's family, external health care professionals and our own employee care partners, you are responsible for coordinating and evaluating all Resident services while ensuring excellent hospitality. You will model a healthy Community culture aligned with the Mission, Vision, and Core Values. You will foster a vibrant community that promotes meaningful relationships, individual choice, and a sense of belonging for all.
The Licensed Practical Nurse oversees all aspects of nursing and care for Residents, based on clinical best practices. As key leaders in the Community, LPNs will work closely with service areas, the General Manager, Supervisors, and Health Services partners to promote well-being through a person-directed approach that honors residents' unique needs and preferences.
Key Accountabilities
Support Health care aides and care aides including but not limited to supporting with resident care needs, assisting with mechanical lifts and transfers, overseeing, observing and document medication assistance, for health care aids and care aides for the MAP competency program. Focus on building trust and a sense of teamwork to ensure exceptional care.
Assist with non-clinical tasks such as laundry, garbage removal, attending resident events, serving meals to ensure excellence in Resident experience. Contribute to a home environment where residents feel comfortable and valued.
Delegate and allocate health care aide and care aide tasks and work assignments as required.
Support urgent relief scheduling of LPNs, health care aides and care aids as required
Leads the response to all emergency codes, medical and nonmedical incidents within the community.
Provide documentation of all incidents, safety concerns, audit results, resident and family concerns, care partner concerns and other regulatory reporting requirements.
Professional documentation of residents' individualized priorities, target dates, strategies, outcomes, interventions, assessments, follow ups, communication to ensure the best possible outcomes for each resident. Ensure that documentation reflects residents' life stories in addition to clinical details.
Complete required audits of charts, wound care binders, medication drawer audits, and bath temperature sheets and others as assigned, to ensure quality and compliance.
Ensures the supply, control, and safekeeping of medications and narcotics, following our wellness strategies and LPN best practices.
Ensures resident supply levels are consistently tracked and requested timely from family or appropriate provider to ensure availability and no disruption to the resident.
Provides medication assistance to the Residents, according to current Wellness Strategies and following regulatory body practices (CLPNA). Include residents in discussions about their medications when appropriate.
Identifies health, safety or maintenance issues, actions and reports issues as required
Ensures appropriate implementation of Infection Prevention and Control practices as per Environmental Public Health.
Ongoing monitoring of the workplace environment and adherence to safe work practices
Provide impartial support, education and excellent relationship building skills to nurture, understand, and respect residents, families, visitors, and care partners. Foster connections that promote a sense of community and joy.
Provide clear and concise feedback and information to Wellness Manager/ Supervisor of observed opportunities for improvement.
On time completion of all annual mandatory education required by provincial regulations and the organization.
Required Education
Grade 12 Diploma or recognized equivalent is required. Must be able to read, write and speak fluently in English.
Graduate of a recognized LPN program
Registered and in good standing with the related Licensing Body CLPNA
Valid Certification in First Aid and CPR.
Current clear criminal record check with vulnerable sector check

Additional Education and Skills
Completion of applicable courses (e.g. Preventing Resident Abuse, Customer Service, Infection Control, Back Care, Dementia, WHMIS)
Current familiarity with the Eden Alternative Philosophy and the Domains of Well-Being is preferred
