SalaryThe salary range for this position is CAD $32.84/Hr. - CAD $41.35/Hr.SummaryReporting to the Patient Care Manager or Resident Care Manager and working in collaboration with the interdisciplinary team, the Licensed Practical Nurse (LPN) is responsible for providing nursing care to an assigned group of patients/residents according to established policies and procedures and the Standards of Practice for the Licensed Practical Nurses of British Columbia. The LPN functions as an independent practitioner, while recognizing situations such as increased patient acuity or care that falls outside the scope of an LPN as appropriate for accessing the RN/RPN for collaborative support and/or clinical intervention.Your duties will include:

  • Assists patients/residents in meeting physical, social, spiritual, and psychological needs
  • Consistent with mission, vision, and values of Providence Health Care (PHC)
  • Implements established protocols
  • Participates in care planning, assessments, interventions, and evaluations
  • Documents information and advocates for patients/residents
  • Attends various meetings including care conferences
  • Assists with orientation of new staff and students
  • Responds to and reports incidents following established procedures
  • Performs clerical duties as required
Qualifications / Skills and EducationEducation:
  • Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training and experience
  • Registration with the British Columbia College of Nurses and Midwives as a practicing LPN registrant
  • Current basic cardiac life support certification
Skills & Abilities:
  • Demonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service
  • Physical ability to carry out the duties of the position
  • Ability to apply decision making and analytical skills
  • Ability to role model conflict resolution strategies
  • Ability to prioritize and organize work
  • Ability to teach and identify patient/resident learning needs

  • Program and Scope: Our hospice/palliative care sites include the following: PHC has a 12-bed in-patient Palliative Care Unit at St. Paul\'s Hospital (10D) and also operates a freestanding hospice, St. John, located on the UBC campus.
St. Paul\xe2\x80\x99s Hospital (10D) *Acute : Palliative care focuses more on acute pain and symptom management needs. Many patients may go home after their time in palliative care or transfer to hospice or long-term care.May\xe2\x80\x99s Place (6 beds) *Sub-Acute : Downtown Eastside, where people have problems with mental health and substance abuse.St. John\xe2\x80\x99s Hospice (14 beds) *Sub-Acute Hospice care is intended for those who are going through their last days of life and are receiving care for end of life.The first community hospice on Vancouver\'s West Side and is situated in a beautiful location on the University of British Columbia\'s campus.Your Day to Day
  • Provide safe and respectful nursing care, adhering to established policies, individualized care plans, and priorities
  • Conduct thorough assessments, monitor signs/symptoms, and communicate with the RN/RPN as needed
  • Prepare patients for procedures, transfers, and admissions, coordinating with the interdisciplinary team
  • Administer medications per protocol, monitor effects, and maintain communication with the team
  • Utilize evidence-based resources to stay updated on nursing practices
  • Respond to incidents such as codes, injuries, spills, and alarms, following procedures
  • Assist with staff orientation, demonstrating procedures and providing support
  • Address patient/resident and family learning needs, provide educational materials, and make referrals
  • Attend care planning meetings, provide input, and advocate for patients/residents
  • Collaborate with the team to develop and evaluate individualized care plans
  • Assist with discharge planning based on patient status and external supports
  • Operate and clean equipment, report malfunctions, and handle hazardous materials. Perform clerical duties such as answering phones, faxing, and maintaining records
