We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing high-quality patient care in various settings, including nursing homes, hospitals, and home care environments. As an LPN, you will play a crucial role in supporting patients' health and well-being while collaborating with healthcare professionals to deliver exceptional care.

Responsibilities



Provide direct patient care under the supervision of registered nurses and physicians. Administer medications and treatments as prescribed, ensuring adherence to safety protocols. Monitor patients' vital signs and report any changes to the healthcare team. Assist with daily living activities such as bathing, dressing, and feeding. Maintain accurate patient records and documentation using electronic health record systems like Epic or Cerner. Support patients with dementia, pediatrics, senior care, hospice care, and pain management needs. Collaborate with interdisciplinary teams for effective case management and patient-centered care. Conduct phlebotomy procedures and laboratory tests as required. Educate patients and their families about health conditions and treatment plans.

Skills



Nursing home experience is highly desirable. Hospital experience in acute care settings is preferred. Familiarity with dementia care practices. Experience in pediatrics or home care settings is a plus. Proficient in patient care techniques and nursing fundamentals. Knowledge of medical terminology essential for effective communication. Competence in medication administration protocols. Experience with electronic health record systems such as Epic or Cerner is advantageous. Background in medical office operations or assisted living environments is beneficial. Understanding of senior care principles and hospice care practices. Laboratory experience related to patient diagnostics is a plus. Skills in pain management strategies are valuable. Strong case management abilities to coordinate comprehensive patient care.
