: Licensed Practical Nurse (LPN) - Admissions and Care Coordinator
Location
: Airdrie, Alberta- Independent Living.
Position Summary
: The LPN - Admissions and Care Coordinator plays a pivotal role in
ensuring a smooth and supportive transition for new residents, supports residents in
maintaining an independent lifestyle and focuses on health promotion and proactively
supports managing their ongoing care needs. This position involves pre-admission
assessments, individualized care plan development, coordination of HCA care, and
when applicable, support in obtaining AHS Client Directed Home Care Funding. The
LPN will act as a primary point of contact for residents and their families regarding
health and wellness, ensuring that residents receive the right level of care at the right
time.
Key Responsibilities:
Pre-Admission Assessment and Planning
Provide pre-move in screening for prospects
Conduct thorough health assessments with prospective residents prior to move-in, including physical, cognitive, social, and functional status. Identify current and potential care requirements, challenges, and preferences.
Support residents to maintain an independent lifestyle that values safety
and quality of life.
Focus on prevention and health promotion
Serves as liaison to family in all matters regarding residents' health and well-being.
Collaborate with the resident and their family to understand their expectations and goals for living in an independent setting.
Provide information and education to prospective residents and families about the services available at the residence or from outside the residence.
Care Plan Development and Implementation:
Develop individualized, person-centered care plans based on assessment findings, resident preferences, and best practice guidelines.
Regularly review and update care plans in response to changes in resident health status, typically (e.g., quarterly or as needed).
Ensure care plans are communicated effectively to all relevant staff, including Health Care Aides.
Care Coordination and Delegation
Determine which care requirements can be met internally by the LPN or HCA team (e.g., medication assistance, personal care assistance, vital signs, personal care oversight, simple wound care).
Identify care needs that may require specialized services and help with access to (e.g., complex wound care, extensive, palliative care, rehabilitation services).
Maintain ongoing communication with resident and possibly with family.
Delegate appropriate tasks to Health Care Aides within their scope of practice and provide clear instructions and supervision.
Residents and Family Support:
Act as a primary resource for residents and their families regarding health and wellness concerns.
Provide health education and support to empower residents to manage their own health.
Advocate for residents needs and preferences within the care team.
Documentation and Compliance:
Maintain accurate, comprehensive, and confidential resident health records and care plans.
Ensure all documentation adheres to professional standards, provincial regulations, and residence policies.
Participate in quality improvement initiatives and maintain compliance with all relevant health and safety regulations.
Collaboration and Communication
Work collaboratively with the General Manager, Recreation team, culinary staff, and other residence personnel to promote a holistic approach to resident well-being.
Participate in team meetings and case conferences as required.
Qualifications
Current and valid registration as a Licensed Practical Nurse (LPN) with the (CLPNA)
Minimum 3 years of experience as an LPN, with experience in geriatrics, community health, or independent living settings strongly preferred.
Demonstrated ability in conducting comprehensive health assessments and developing individualized care plans.
Strong understanding of the scope of practice for LPNs and Health Care Aides.
Knowledge of local care services and referral processes.
Excellent interpersonal, communication (written and verbal), and organizational skills.
Strong written and verbal English communication skills
Ability to maintain confidentiality
Ability to work independently and as part of a team, demonstrating sound judgment and critical thinking skills.

Current First Aid with CPR
WHMIS Certification

Pay: $26.00 per hour
