Licensed Practical Nurse

Sylvan Family Health Centre is currently hiring a Full Time LPN to join our family practice.

We are a teaching facility, with a focus on patient centered care. You will be a key participant in the evolution of patient care, within Sylvan Family Health Centre. You must be energetic and be willing to be directly involved in patient care, in a fast-paced medical environment.

The ideal candidate must be forward thinking, energetic, have a strong work ethic and be self-motivated professional to join our team as we embark on clinical development to enhance patient care. You must be willing to take the initiative to improve patient care and be an active part of quality improvement initiatives. The ideal candidate must be able to work in a team environment and have the ability to adapt to change.

Specific duties include, but are not limited to: The leadership of other nursing and medical office assistant staff, administration of medications, injections - including vaccines; initiate triage and assist in treatment of priority 1 patients (R/O MI, Allergies, Etc.); create & review Complex Care Plans; Prenatal Patient Protocol - including initiate history, screening and 1 week and 6 week postnatal visit checks; Long Term Care physician/patient liaison; Post Hospitalization Discharge; Diabetic Foot Exams; Wart Treatments; Wound Care; DMED Screening; Goals of Care; Same Day Protocols - including UTI, sore throat, influenza like symptoms as an example only. The ideal candidate must be able to handle difficult nurse emails and phone calls. Maintain and fax vaccine reports and inventory. You must be able to share in clinical care of our patients, as your roles within the clinic will be constantly evolving.

You must be an LPN in good standing.
