Job Description

Are you a compassionate and skilled Licensed Practical Nurse (LPN) located in the Spruce Grove/Stony Plain area, looking to make a meaningful impact in the lives of clients? We are seeking a dedicated LPN to join our home care team, providing high-quality care while fostering strong relationships with clients, families, and healthcare professionals.

The Licensed Practical Nurse (LPN) will provide compassionate and skilled nursing care to clients while working both independently and as a part of an interdisciplinary healthcare team. The LPN will collaborate closely with AHS case managers, doctors, and family members to ensure comprehensive care and the continuous development and revision of personalized care plans. Key responsibilities include conducting Monthly Quality Assurance visits, monitoring daily progress notes (charting), and ensuring compliance with care standards.

The LPN will also play a vital role in the shared supervision of health care aides and caregivers, in collaboration with the Client Care Coordinator and Operations Manager. The LPN will delegate tasks to Health Care Aides and Caregivers, ensuring efficient and effective delivery of care, and will provide guidance and support to the team as needed. The role requires a detail-oriented individual with strong communication and organizational skills, committed to maintaining the highest standards of client care.

The ideal candidate will have strong clinical skills, the ability to work independently, and have a passion for building meaningful relationships with clients and their support teams.

Work Hours:

This is a unique position that is part time with a flexible schedule. Hours of work fall mostly between Monday to Friday.

Location:

Remote (from home) or various locations (clients homes) in the Spruce Grove, Stony Plain and surrounding areas.

Duties:

The following list of duties is a guideline of possible duties to be performed independently or as a group of duties in the course of working with the client. The LPN duties are not limited to those listed:

1. Collaborating with healthcare professionals, including AHS case managers, doctors, and family members to support client health and well-being

2. Creating, implementing and revising individualized care plans based on client needs.

3. Conducting monthly Quality Assurance visits to ensure care standards are met.

4. Overseeing daily charting, including daily progress notes and task completion, to maintain accurate client records and observe any changes in client's health.

5. Providing joint supervision of health care aides and caregivers alongside the Client Care Coordinator and Operations Manager.

6. Delegating tasks to health care aides and caregivers to ensure efficient and effective care delivery.

7. Is able to maintain confidentiality of information.

8. Works as a team member.

9. Is able to perform the following physical activities: Kneeling, Stooping, Standing, Reaching, Pulling/pushing, Walking, Grasping, Moving fingers, Hearing, Talking, Sitting, Repetitive motions.

Qualifications:



1. Current LPN license with CLPNA in good standing.

2. 2+ years of experience in nursing.

3. Experience in home care, geriatrics, chronic disease management, wound care and medication administration is preferred but not required.

4. Current certificate in standard first aid and CPR.

5. Valid driver's licence and reliable transportation.

6. Ability to work independently and as a part of a team.

7. Excellent communication and listening skills.

8. Strong organizational, time management and problem-solving skills.

9. Compassionate, patient and caring nature.

Job Types: Part-time, Permanent

Pay: From $27.00 per hour

Schedule:

Day shift Monday to Friday Weekends as needed
