Job Summary:
Dover House is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team of RNs, LPNs, and HCAs, working night shift, 2-3 nights a week, with the goal to have every other weekend off if able. As an LPN, you will play a vital role in providing high-quality patient care and ensuring the well-being of our patients. Responsibilities:
- Provide direct patient care, including administering medications, monitoring vital signs, and assisting with activities of daily living
- Collaborate with the healthcare team to develop and implement individualized care plans
- Document patient information, treatments, and observations accurately and timely
- Assist in the coordination of patient appointments, referrals, and follow-up care
- Educate patients and their families on health conditions, medications, and self-care techniques
- Maintain a clean and organized work environment
Qualifications:
- Valid LPN license with BCCNM
- Possess maturity with good physical and mental health, empathy, patience and tact
- Must be able to lead and supervise care personnel
- Multi-tasker and excellent organizational, problem solving, and communication skills
- Able to work well with others and be a team player, as well as able to work independently
- Compassionate and caring nature with an excellent understanding of persons centered care
- Provide care to residents in accordance to Dover House's philosophy, goals and objectives
- Assist residents to meet physical, emotional, and psychological needs to facilitate independence
- Proven experience in patient care, preferably in nursing homes, medical offices, or home care settings
- Knowledge of dementia care, cardiovascular and geriatric health needs
- Excellent understanding of the English language, in both written and verbal skills
- Able to take initiative and be proactive with care for the seniors
- Excellent communicator with a solid foundation of: various disease processes, medications, and medical challenges
- Punctual, reliable, loyal, and dedicated
- Ability to multitask and prioritize tasks effectively
- Compassionate and empathetic nature
