ON BOTH POSITIONS (DAY-SHIFT,NIGHT-SHIFT ) THE LPN WILL BE :

Reporting to the Director of Care (DOC), the Licensed Practical Nurse (LPN) is responsible for providing planned and resident and family centred care in accordance with the provincial standards and with Lynn Valley Care Centre standards, policies and procedures.

Key Responsibilities:

To complete and ensure residents' assessment, care plans and documentation are up to date and changes in residents' conditions are communicated to DOC and physicians To administer medications adhering to best practice guidelines To plan, coordinate, manage and evaluate the resident's care plan and working with the resident/substitute decision maker, physician, nursing and contracted service providers To act as advocate for the residents and facilitate resident/family education and discharge planning To participate in resident's care conference To respond to and manage significant changes in residents' condition and attend to medical emergencies To respond to resident/family concerns and engage appropriate action taken and communicated to DOC and physician To uphold and promote the values of Lynn Valley Care Center and to uphold the organisation's Code of Conduct To provide care in accordance with established standards, and the mission, vision, values, policies and procedures of Lynn Valley Care Centre

Required Qualifications:

Current registration as a Licensed Practical Nurse with the province of British Columbia Experience working with seniors and unregulated care providers Education and experience in gerontology, rehabilitation and long term care an asset Must possess strong written and oral communication, critical thinking, care, coordination, time management and resident focus skills Must have demonstrated sound clinical knowledge and ability to work and provide support to unregulated care providers Current Basic Cardiac Life Support and First Aid certification required
Job Type: Part-time

Pay: $54,600.00-$54,700.00 per year
