Job Description

Charaka Rehab is currently seeking a Licensed Practical Nurse (LPN) to provide in-home care services in the community. This is an opportunity to work in a supportive and flexible environment focused on client-centered care.

Responsibilities:

Provide direct patient care to a senior client at his home. Administer medications and treatments as prescribed by healthcare professionals Monitor patient vital signs and report any changes or concerns to the appropriate medical staff Assist with patient assessments and develop care plans in collaboration with the healthcare team Perform routine procedures such as wound care, catheterization, and specimen collection Document patient information accurately and maintain medical records Provide emotional support and education to patients and their families Collaborate with case managers to coordinate patient care and ensure continuity of services

Licensed Practical Nurse / LPN Must Haves:

Graduation from a recognized Practical Nurses program or equivalent combination of education, training, and experience recognized by the BCCNM. Holds an active registration with the BCCNM as an LPN. Maintain Occupational First Aide Level 1 status. Passes and complies with Ministry of Justice Criminal record Check upon hire and every 5 years thereafter. Ability to effectively communicate in English, both verbal and written.

Skills:

Experience working in medical office or hospital settings Strong patient care skills with the ability to assess, plan, implement, and evaluate nursing interventions Knowledge of medication administration procedures and ability to safely administer medications Excellent communication and interpersonal skills We are looking for a licensed practical nurse who is compassionate, detail-oriented, and dedicated to providing high-quality patient care.
