We are looking for a Licensed Practical Nurse (LPN) to join our team on a relief/casual basis, or on-call.
LPNs are an integral part of our Home Medication Management Program to provide our clients with the outreach, support and care they need. We serve the elderly, chronically-ill, and at-risk populations. If you are not comfortable with visiting clients in recovery from substance use disorders at their homes in social/supportive housing please do not apply.
We operate 7 days per week and shifts start at between 7:30 and 8:30am. Shift length varies but expect 6-7 hours.

Wages are negotiable depending on experience and shift premiums apply. There is a mileage reimbursement for the use of your personal vehicle. Shortlisted candidates will be contacted for an interview.
Responsibilities:
Visiting patients in the community in their homes
Administering medications to patients (oral, injectable, topical, transdermal patches)
Medication deliveries to residential support workers, following up on medication compliance and collaborative problem solving with other support workers and patient's family members.
Performing patient specific assessments (ie: Blood glucose and blood pressure monitoring)
Noting changes in behaviour for clients with mental health or substance use disorders
Maintaining up to date patient records by documenting all interventions and assessment data and submitting daily reports to the supervising pharmacist

We are looking for LPNs with the following qualities:
Highly motivated with a strong work ethic
Caring, friendly attitude and strong communication skills
Confident and comfortable with driving to client addresses
Enthusiasm to contribute positively in the community in a healthcare setting
Ability to adapt to a rapidly changing work environment
Understanding and non-judgmental attitude toward behaviours that put individuals at risk
An understanding of Harm Reduction practices and willingness to practice in a harm reduction environment
Requirements:
Driver's License and consistent access to your own vehicle with adequate insurance coverage
Currently in good standing with BC College of Nurses and Midwives
Criminal Record Check will be requested upon successful offer
