Detailed Overview:
Provides an accessible, client-centered and integrated approach to the processing of requests for Home Health services; performs initial client consultation, assessment and intervention, utilizing standardized assessment tools, and based on the client's needs, urgency of request and best practices; screens referrals and confirm client's eligibility for services; prepares and maintains client records and provides intake/referral data and statistics; participates in discharge planning, team meetings, continuing education and engages in on-going team planning and quality improvement.
Responsibilities: Reviews, prioritizes and processes requests for home-based, ambulatory or other community-based services from potential and actual clients and other healthcare professionals; gathers data and determines priority of service provided to the client including risk level of service request; obtains additional information for medical orders and coordinates service supports as required.
Accepts and confirms referrals for eligibility for service. Utilizing standardized tools, assesses the client to determine eligibility for services based on established standards; determines initial interventions and response time needed; recommends community and/or other community-based services and collaborates with the interdisciplinary team to coordinate and prioritize referrals based on client needs.
Coordinates the flow of care by liaising/interviewing/providing advice on a range of health care and community professionals and organizations.
Provides education and counseling to clients/family/caregivers and provides guidance to other caregivers and health providers regarding intake policies, procedures and processes, health care resources and the availability and/or eligibility for services.
Participates in discharge planning and communicates and consults with appropriate members of the health care team regarding the care and support plan to ensure continuity of appropriate care for the client.
Maintain and completes required documentation; compiles information and data as required according to established procedures; prepares statistical information, correspondence, reports and other documentation, as required.
Participates in interdisciplinary care conferences, in-services and team meetings as required; demonstrated and provides collegially sharing of information with the interdisciplinary team.
Performs other related duties as assigned.
Qualifications:
Education and Experience
Graduation from an approved program of Practical Nursing, plus two (2) years practical nursing experience, including a minimum of one (1) year community health nursing and/or home support experience.
Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
Skills and Abilities
Knowledge of gerontology, geriatrics and the needs of adults living with chronic illness.
Ability to complete client assessments using relevant assessment tools.
Knowledge of broad health care services, community access and the continuum of care.
Knowledge of relevant acts, legislation, standards and policies governing access to and the operation of long term care homes, assisted living facilities, and other community resources.
Knowledge of and ability to utilize applicable clinical information systems (e.g. Gold Care, Strata Pathways, Paris, Meditech ITS).
Ability to use Excel, Adobe Pro, Microsoft Outlook and other relevant software applications.
Ability to establish and maintain rapport with clients.
Ability to observe and recognize changes in clients.
Ability to work independently and as a member of a team.
Ability to communicate effectively both verbally and in writing.
Ability to plan, organize and prioritize work and client care plans.
Ability to type 40 wpm.
