Position Summary
The Licensed Practical Nurse (LPN) provides comprehensive nursing services to the patient population within Mosaic PCN clinics. The LPN assists patients by providing appropriate assessments and documenting accordingly. The LPN works closely with physicians and the multidisciplinary team to develop and implement care plans, evaluate outcomes, and provide follow-up with patients.
The LPN works to ensure a high quality of evidence-based health care is provided in accordance with the standards defined by the governing Provincial College (CLPNA). The LPN works to their full scope of practice with an emphasis on providing comprehensive care, education, and guidance for the patients.
Key Responsibilities
Assessment
Performs and contributes to the initial and ongoing nursing assessment of assigned patients, by collecting and analyzing information from multiple sources.
Assesses and identifies real and potential risks at the bio-psycho-social-cultural-spiritual levels.
Identifies and analyzes the assessment data to participate and collaborate in selecting established nursing diagnosis.
Intervention
Uses critical thinking and clinical judgement to coordinate and facilitate the care management of patients.
Reviews the health information including diagnosis and treatment plans for the patients and responds to patient questions and concerns.
Implements both prescribed interventions (e.g., diagnostic test, medications) and performs independent nursing interventions (e.g., patient education, identifying and explaining the link between health promotion strategies and health outcomes).
Utilizes health coaching/case management approaches when assessing, planning; implementing, and evaluating care needs of the patient.
Utilizes motivational interviewing techniques and behavioural change management to provide support, advice, and additional resources to empower patients to achieve goals in health behaviour changes.
Provides comprehensive assessments, health screening and facilitate interventions and referrals to other health care providers as appropriate.
Teaches, councils, and advocates to enhance health and well-being with an emphasis on prevention of disease.
Participates in quality improvement development at the clinic level.
Monitors the assessment data, the ongoing health status of the individual/family, recognizes changes in health care needs, and adjusts the care plan accordingly.
Empowers patients while recognizing challenges related to vision, hearing, speech and language, and adapts care to minimize the challenge while promoting patient health and independence.
Multidisciplinary Collaboration
Collaborates with other members of the nursing team and consults with other healthcare providers.
Understands own role and the roles of other professions, using this knowledge to appropriately establish and meet patient goals.
Comprehends the principles of team dynamics and group processes to enable effective multidisciplinary team collaboration.
Communicate with practitioners from different professions in a collaborative, responsive, and responsible manner.
Mentor's colleagues and students in areas of expertise and seeks mentorship to achieve full potential in professional development.
Documentation
Documents all phases of patient care in the appropriate Electronic Medical Record (EMR) to maintain accurate and timely records, as well as recording the details of clinical interactions in MPCN evaluation systems.
Community Engagement
Participates in community activities within the Mosaic PCN catchment area, including health fairs, conferences, and other community events that promote Mosaic programs and services.
Professional Development
Identifies and works to achieve own professional development needs which support goals set in performance reviews.

Qualifications
Education & Licenses
Diploma in Licensed Practical Nursing
Licensed with College of Licensed Practical Nurses of Alberta (CLPNA).
Experience
A minimum of 2 years nursing experience
Experience working with ethnically diverse, vulnerable, and high-risk populations.
Knowledge, Skills and Abilities
Effective communication skills both verbally and written in English.
Knowledge of primary health care and motivational interviewing.
Ability to work independently and build a good working relationship with multidisciplinary team, patients, and other members of the community.
Computer literacy including Microsoft Office suite, Outlook calendar/email management, and Electronic Medical Records (EMRs).
Demonstrates critical thinking skills, including clinical judgement and problem-solving skills.
Strong time management and organizational skills.

Occasional evening and weekend hours required.
Ability to travel on public transportation or valid drivers license with access to own car in good repair is required.
