CND OHT Lead, Population Health and Special Projects



(40 hours/week, Permanent)



The Cambridge North Dumfries Ontario Health Team (CND OHT) comprises 40+ agencies and health services providers from Cambridge, North Dumfries and the Waterloo Region. Langs' is the host organization for CND OHT employees and so the successful candidate will be an employee of Langs.

What We are Looking For:



The Lead, Population Health and Special Projects will support the project work of the CND OHT This role will provide project management support, as well as strategic guidance to support the sustainability of the implementation of CND OHT initiatives. This position will involve close collaboration with members of the CND OHT staff support team, leaders across the CND OHT, primary care clinicians and external partners.

Qualifications:



University degree in in a related field (i.e. public health, health promotion, health sciences, sociology or health/business administration) or an acceptable equivalent combination of education and experience

Experience in project management, process/quality improvement and/or population health is an asset

Experience in clinical settings, especially in working with equity-deserving communities, is an asset

Experience in supporting programmatic design and development, especially in health care

Ability to resolve conflict, apply an entrepreneurial approach to complex problem solving, create plans and materials where few parameters are provided

Demonstrated written and oral communication skills

Ability to work under pressure and tight timelines

Ability to maintain privacy

Proficiency in Microsoft Office

Ability to work flexible hours

Position Duties and Responsibilities:



Manage the planning and implementation of various population health management projects in support of the CND OHT priority populations and primary care priorities. Duties will include preparation and maintenance of project documentation, facilitation/co-facilitation of project team meetings, on-the-ground project operational readiness.

Lead relationship management amongst project partners and external stakeholders to support the implementation and sustainability of CND OHT population health initiatives.

Participate in Ontario Health webinars and communities of practice on behalf of the CND OHT, as required, to ensure alignment and collaboration in the areas of care transformation, navigation and coordination.

Work Arrangement



:Reporting to the CND OHT Director, the Lead will primarily work remotely, with occasional in-person meetings or travel within the Waterloo Region as required. What We Can Offer You:

The successful candidate would be eligible for the following benefits:



Starting Salary of $79,595 to $88,021 annually

3 Weeks paid vacation, with an increase to 4 weeks after 1 year of employment

15 Sick/Personal Days per year

10 Professional Development days per year

Enrolment in Healthcare of Ontario Pension Plan (HOOPP)
