We are officially starting our recruitment for the Black Health and Social Services Hub Canadian Mental Health Association Peel Dufferin (CMHA Peel Dufferin) is partnering with LAMP Community Health Centre (LAMP CHC) and Roots Community Services Inc (RootsCS) to design, develop and implement an integrated health and social services hub with and for the Black, African and Caribbean (BAC) communities in Peel Region. We are looking for Two Intensive Case Managers to join our team! The Intensive Case Managers will report to the Program Manager, BAC Multidisciplinary Team and provide case management supports based on needs assessments and care plans to individuals experiencing serious mental illness and addictions concerns. The Intensive Case Managers will work in collaboration with internal and external programs and agencies, while working within a holistic, integrated model of care to address client\xe2\x80\x99s unmet needs using a stepped care approach**. **Stepped Care approach is a system of delivering and monitoring mental health and addictions treatment so that the most effective, yet least resource intensive treatment, is delivered first, only \xe2\x80\x9cstepping up\xe2\x80\x9d to intensive/ specialist services as required and depending on the level of client distress or need. We recognize the importance of allyship and the critical role allies play in affecting change. Conduct needs assessments, develop, and implement comprehensive care plans and ongoing monitoring until discharge
Provide contracted case management, task-based interventions and activities of daily living related to social determinants of health
Assess and provide crisis support and plans; make connection to crisis services when appropriate
Utilize multidisciplinary team to build a collaborative care plan and consult with other disciplines as needed
Refer and link clients to community resources and groups that support recovery goals
Manage a caseload including regular re-assessments, updating risk levels and caseload reviews
Participate in regular staff meetings and care planning to provide case management expertise for client care within multi-disciplinary team
Facilitate groups, including the creation of new groups as needed and acting as a back-up facilitator
Contact with primary care at regular intervals to report clients\xe2\x80\x99 care plan and discharge summary
Work collaboratively with other Case Managers to support organizational needs(e.g., volumes across all regions) covered by CMHA PD
Update all appropriate documentation within 24 hours of client contact, as required by CMHA PD policy and procedure
Participate in the Case Management Community of Practice and other agency and community committees and workgroups as requested by manager.
What you need to join our team
Post-secondary degree or diploma in relevant discipline (e.g., BSW, MSW, OT, RN)
Being a registered and / or regulated health professional is an asset
Expertise in navigating community mental health and/or community based human services
Minimum 2 years of experience with a focus on assessment, brief interventions and short term supports
Minimum of 2 of the following training and willingness to train up in 6 months:

Demonstrated cultural sensitivity and ability to effectively work with Black, African and Caribbean communities.
Demonstrated experience working within a multidisciplinary team and evidence of an understanding of the importance of partnerships and collaboration.
Strong understanding of the current and historical health inequities experienced by Black, African and Caribbean communities.
Demonstrated and practical understanding of anti-Black racism and intersectionality, and various barriers which affect diverse identities
Intersectional understanding of current social realities and barriers experienced by BAC communities, from an anti-oppressive, anti-racist, feminist, self-aware and trauma-informed approach to practice is mandatory.
Knowledge of local cultural programs and services to refer individuals and families from Black, African and Caribbean communities within the service catchment area.
Being knowledgeable in a variety of evidence-informed treatment modalities including anti-Black racism, anti-oppression and other culturally sensitive approaches to intervention.
Demonstrated cultural competence and ability to work with diverse groups as well as an understanding of social determinants of health and impact on vulnerable communities
Skilled at engaging in matters that require sensitivity and empathy across a diverse community, excellent judgement, and discretion; ability to operate in a culturally sensitive and anti-oppressive framework
Valid Canadian driver\xe2\x80\x99s license and access to an insured automobile in good repair A satisfactory Vulnerable Sector Screening (Police Check) report
