Job Description

Better Living is seeking a compassionate, organized, and adaptable individual to join our Social Work team as a Case Manager. This is a temporary (1-year) position, with the possibility of extension or becoming permanent. This role is designed to support clients--primarily older adults and individuals living with disabilities--by helping them access and navigate health and social services based on their unique needs.

The Case Manager works closely with clients, caregivers, and professionals across the health and community sectors to ensure timely, coordinated, and person-centered support. Depending on organizational priorities and funding, this role may shift focus between intake, care navigation, case management, and care connector functions.

This position reports to Manager, Social Work and Navigation Services

The successful candidate for this position must have the following qualifications:



Minimum diploma in Social Service Work (SSW) from an accredited institution Bachelor of Social Work Degree (BSW) is preferred Registration in good standing, with the Ontario College of Social Workers and Social Service Workers required Minimum 1 year of experience (employment or placement) in social services or community health Familiarity with health care and social service systems in Toronto and surrounding areas Demonstrated ability to support diverse populations including seniors and individuals living with disabilities Experience using electronic client databases and referral systems (Caredove, AlayaCare) preferred Ability to work both independently and collaboratively in a fast-paced environment Excellent communication, organization, and problem-solving skills Second language considered a strong asset Successful completion of a Vulnerable Sector Screening

The successful candidate(s) for this position must be able to:



Conduct needs-based assessments with clients, caregivers, or referring agencies to determine appropriate supports and services. Provide information, education, and referral assistance to clients navigating community, health, and social service systems. Offer guidance to clients and families during care transitions, times of vulnerability, or when support systems are fragmented. Collaborate with internal teams and external partners to coordinate services and follow up on referrals and care plans. Maintain accurate, up-to-date client documentation in electronic records. Support service continuity by identifying service gaps and advocating for clients where appropriate. Participate in program development, quality improvement, and community outreach as needed. Provide general support to the Social Work department, including assisting with security check-ins, friendly visiting, and community inquiries when required.
