The Infection Prevention and Control (IPAC) department is an integral part of Trillium Health Partners. The IPAC Hub is a provincial initiative to provide IPAC support to congregate living settings through hospital outreach. As a member of this team, the IPAC practitioner facilitates infection control activities and is considered an agent of change through the interpretation of standards providing best practice recommendations with the primary goal of reducing the risk of healthcare associated infections (HAI) in long-term care and retirement homes and other congregate living settings in Mississauga and South Etobicoke, through consultation and onsite assessment and education/coaching.

The successful IPAC practitioner will directly report to the Manager Community Outreach IPAC Hub and will be expected travel to locations in Mississauga and South Etobicoke.

Responsibilities:

  • Participates in the development, communication and implementation of IPAC, current best practice guidelines and applicable standards, and policies and procedures in particular for congregate living settings
  • Provides consultative advice, feedback and report to external stakeholders regarding surveillance data, investigations, audits and issues of IPAC significance
  • Provides ongoing IPAC education to staff, physicians, students, volunteers, essential caregivers, and residents
  • Participates in quality improvement initiatives and supports congregate living settings to implement infection control recommendations based on the latest scientific evidence from Public Health Ontario/Provincial Infectious Diseases Advisory Committee Best Practice documents
  • Employs a train-the-trainer approach to conduct auditing, training, coaching, and education and provides on the spot and formal feedback through reports
  • Develops relationships with Hub partners (including congregate living settings, public health units (PHUs), Ontario Health Central Region/Mississauga Halton, Home and Community Support Services (HCCSS) and supports all levels of staff, leaders and professional staff with IPAC expertise
  • Responds to requests for IPAC support from PHUs and Partners or directly from facilities in outbreak management
  • Other duties may be assigned based on needs and as identified via risk mitigation strategy
Qualifications:
  • Minimum of 3 years\xe2\x80\x99 experience in Infection Prevention and Control in a non-acute care setting or a diverse acute care setting which includes a long-term care service
  • Current certification with the Certification Board of Infection Control and Epidemiology (CIC)
  • Degree in health related discipline (e.g. nursing, laboratory, epidemiology, and other allied health) or equivalent combination of education and experience with commitment to achieve CIC within one year of employment with THP
  • Demonstrates strong IPAC knowledge and experience in related areas of infectious diseases, microbiology, epidemiology, statistics, asepsis; disinfection/sterilization; outbreak management, HVAC & construction
  • Demonstrated skills in coaching, mentoring and excels in adult education
  • Strong written/verbal communication skills
  • Critical thinking in complex and stressful situations, conflict resolution skills, high emotional intelligence
  • Proficiency in the use of computer databases, spreadsheets and literature search, familiarity with EPIC HIS is an asset
  • Ability to work independently, self-directed and able to effectively lead and provide support to clinical & non-clinical teams to manage complex and challenging situations
  • Demonstrated ability to work collaboratively in multidisciplinary team environment
  • Self-directed, well-organized and detail-oriented
