INDEPENDENT LIVING TRANSITION FACILITATOR / FAMILY ENHANCEMENT FACILTATOR

The Independent Living Transition Facilitator (ILTF) is responsible to role model and teach independent living skills to youth. Youth will be supported and empowered with the help and guidance of the ILTF to reach independence and autonomy.

Duties and Responsibilities:

• Mentors and role models vital life skills for successful independent living including budgeting, paying bills, house chores and cleaning, etc.;
• Teaches and assists with grocery shopping, healthy meal planning and food preparation;
• Provides creative and hands-on learning opportunities that reflect goals and objectives outlined in the individual case plan;
• Facilitates access to existing supports and navigate resources in the community, such as education resources, income security, housing supports, employment services and training, health and mental services, legal services, etc.;
• Establishes safety and well-being practices, including safety plans;
• Transports and accompanies youth on outings such as appointments, shopping, and leisure activities;
• Builds positive relationships and rapport with youth, helping to identify personal strengths to promote growth, self-worth, and a sense of belonging;
• Completes clear, concise and accurate documentation including written reports and financial reconciliations;
• Reinforces conflict resolution, positive communication, and household respect through positive reinforcement and supportive listening;
• Engages in behaviour management, intervening in crisis situations, according to the guidelines and framework of Marymound's policies and procedures, the youth's safety plan, NVCI and ASIST;
• Attends staff meetings, treatment conferences, and training as required.
• Implements strategies such as planned daily activities, coordinated treatment interventions, structured environments, and organized recreational and social activities;
• Demonstrates an active commitment towards professional growth and advancement through participation in training, workshops, etc.
• Other duties as assigned.

Qualifications:

• Experience providing physical, psychological, and social support to at-risk youth or other vulnerable populations is required;
• Must be familiar with the developmental, educational, emotional, social and recreational needs of children and youth;
• Certificate/diploma in Child & Youth Care, or applicable post-secondary education (i.e. social work, psychology, counselling, etc.) is preferred;
• Valid First Aid/CPR is required;
• Mental health/crisis training certifications including NVCI (Non-Violent Crisis Intervention) and/or ASIST (Applied Suicide Intervention Skills) would be considered an asset;
• Valid class 5I (intermediate) driver's license, and satisfactory driver's abstract;
• Access to a reliable vehicle is required;
• Satisfactory Criminal Record and clear Vulnerable Sector checks;
• Clear Child Abuse Registry and satisfactory Prior Contact checks;
• Comprehensive knowledge of children/youth services and resources available in the community;
• Knowledge of Indigenous culture, or willingness to learn, including participating in cultural practices, programming and ceremonies alongside youth;
• Excellent written and oral communication skills;
• Flexibility and creativity to identify and adopt new ways to do things;
• Ability to work independently with minimal supervision;
• Proficient computer knowledge and experience with Microsoft Office software;
