Duties

Conduct assessments to determine client needs and develop individualized service plans. Provide guidance and support to clients in accessing community resources and services. Maintain accurate records of client interactions, progress, and outcomes using databases such as Access and SharePoint. Collaborate with other professionals, agencies, and community organizations to coordinate services. Facilitate workshops or support groups as needed to empower clients. Ensure compliance with all relevant policies and regulations regarding case management. Utilize strong administrative skills to manage documentation and reporting requirements efficiently. Exhibit leadership qualities by mentoring junior staff or interns when necessary.

Experience

Proven experience in case management or a related field is preferred. Strong organizational skills with the ability to manage multiple cases simultaneously. Proficiency in records management and familiarity with office software, including Microsoft Office Suite. Experience working with diverse populations; bilingual abilities are a significant advantage. Excellent written and verbal communication skills are essential for effective client interaction.

Job Types: Full-time, Permanent

Pay: $17.20-$20.00 per hour

Expected hours: 40 per week

Monday to Friday Weekends as needed
