Responsibilities
Provide
goal-directed, patient-oriented nursing care
aligned with professional standards.
Communicate effectively with the healthcare team, sharing ideas and concerns, and actively participate in problem-solving.
Use clinical knowledge, skill, and judgment to
triage incoming patient calls
, assess health needs, and provide appropriate guidance.
Demonstrate understanding of
chronic disease management
and
patient self-care strategies
.
Initiate and update
Electronic Medical Records (EMR)
for patient interactions, including visit purpose, history, and health needs.
Accurately
record patient vitals
when applicable and ensure readiness for virtual or in-person consultations.
Maintain
confidential, concise EMR documentation
for patient follow-ups and preventive care recalls.
Exhibit
excellent phone etiquette
and empathetic communication skills.
Participate in team meetings and contribute to improving organizational operations.
Ensure all medical information provided is
current, evidence-based, and unbiased
.
Act as a
clinical resource
for administrative staff, MOAs, and other disciplines.
Qualifications
Minimum
1-2 years of nursing experience
(clinical or telehealth).
Proficiency in
data entry
, EMR systems, and
Microsoft Office
.
Ability to work flexible shifts and collaborate effectively in a team environment.
Strong
time management
, punctuality, and reliability.
Attendance record in good standing.
Nice to Have
Experience in
telemedicine or virtual care settings
.
Background in
family practice or preventive care programs
.
Familiarity with
chronic disease prevention strategies
and patient education.
