HART Hubs are community-based care hubs designed to address mental health, addiction and housing needs for people experiencing homelessness. They provide integrated care, offering a wide range of services such as primary care, mental health services, addiction care and employment supports. With targeted community outreach and rapid referral pathways, HART Hubs aim to support individuals with complex needs, helping them recover, achieve housing stability, and inclusion.
Services and Housing In the Province (SHIP) is pleased to partner with (CMHA Peel Dufferin) and various agencies in the Peel Region, to offer services to the local community at the new Brampton based Homelessness and Recovery Treatment (HART) Hub.

As part of a multi-disciplinary team, the Housing Support Worker provides housing case management supports to individuals experiencing homelessness or are at risk of becoming homeless, including engaging with landlords, identifying opportunities for housing, and supporting clients to secure and maintain safe housing.
Key Duties and Responsibilities
Conduct comprehensive standardized housing assessments (SPDAT), including housing readiness and need for supportive housing
Provide housing guidance and on-site housing support, including life skills development, budgeting assistance, and community building
Liaise with landlords to access private market housing opportunities for clients that are safe, well maintained and affordable
Connect individuals with housing, including accompanying them to view potential housing options and being present on move-in day
Regularly assess for risk and respond with mitigation strategies, including crisis intervention and de-escalation supports and connecting to crisis services when appropriate
Refer and link clients to community resources and groups that support housing goals
Facilitate recovery and/or social-recreational groups as needed, including developing new group materials
Actively participate in staff meetings, team huddles, supervisions, case conferences, trainings, and events as required Update all appropriate documentation within 24 hours of client contact
Adhere to agency Health & Safety Program and Client Safety Framework
+ Demonstrated experience working within a multidisciplinary team and evidence of an ability to collaborate and partner
+ Working knowledge and/or an understanding of various disciplines; including but not exclusive of mental health, substance use, harm reduction, newcomer challenges and barriers faced by those in marginalized populations
+ Demonstrated cultural competence, ability to work and apply practical understanding of different elements of diversity, equity, inclusion and belonging as well as an understanding of social determinants of health and impact on vulnerable communities
+ Knowledge of a variety of evidence-informed treatment modalities including motivational interviewing and trauma informed practices
+ Experienced at engaging in matters that require sensitivity and empathy across a diverse community, excellent judgement, and discretion; ability to operate with various frameworks including culturally sensitive, anti-racist, anti-oppressive, 2SLGBTQ+ affirming and intersectional
Perform additional duties as required/delegated by Management
Qualifications
Bachelor's Degree or Diploma in Social Work, Social Services, Health Services, or equivalent
2+ years' experience providing housing support to individuals with mental health challenges
Proficiency in the Residential Tenancies Act and Landlord/Tenant Board legislation and RGI knowledge
Understanding of the Housing First philosophy
Understanding of homelessness and at risk of homelessness
Excellent time management, prioritization abilities, and communication
Strong working knowledge of Microsoft software (e.g. Excel, Word, etc.)
A valid Ontario Class "G" Driver's license and a vehicle with adequate insurance coverage is required
Valid certification in CPI, CPR, First Aid, and Assist is mandatory
Reporting Function:
The Housing Support Worker reports directly to the Manager, Supportive Housing. They work with both internal and external partners and will interact with all levels within the organization. They have no supervisory responsibilities. They are a support system, resource, and an advocator for the team and Organization.
Working Conditions and Requirements:
Rotational shifts (12 hours) including day, afternoon and weekends (9am - 9pm)
Able to sit/stand for prolonged periods of time (building inspections, site meetings, unit inspections, etc.)
Computer screen use (multiple screens if applicable) for prolonged periods of time
Able to lift up to 20lbs (product, devices etc.)
Vehicle with adequate insurance coverage to travel to community sites is required
You may be required to administer naloxone in response to suspected opioid overdoses, and must complete appropriate training to recognize overdose symptoms, safely administer naloxone via nasal spray or injection, and follow emergency protocols including contacting emergency services. You will receive training within the first eight weeks of employment.

Shifts are 12 hour shifts through a 6 week rotational schedule
9:00am - 9:00pm rotational 3 days weekly (Monday - Sunday schedule based upon shift rotation)
37.5 hours/week - Union
