FTE/Hours of Work: 1.0 FTE - 2015 annually; 7.75 hrs/day

Position Summary:



The Concordia Village 4 (CV4) Housing and Supports Worker (HSW) directly supports participants living with mental illness in an integrated community setting. The HSW will support participants to achieve their recovery goals and reduce the risk of eviction. The HSW will also assist in carrying out participant support plans with a strong emphasis on Activities of Daily Living (ADL's). The HSW will engage in crisis intervention and assist participants with development and/or maintenance of support networks. The HSW will use Recovery Principles in carrying out their regular duties.

Qualifications:



Post-secondary education in Human Services, other related disciplines, and/or equivalent experience required.

Experience in facilitating group learning and leisure activities an asset.

Knowledge, skills, and ability:



Basic understanding of different psychiatric conditions.

Excellent crisis intervention and conflict mediation skills, with ability to remain calm in crisis.

Personal qualities of empathy, understanding, patience, and flexibility.

Excellent rapport-building skills.

Ability to maintain positive attitude.

Principle Duties and Responsibilities:



Planning and facilitating group activities/events.

Working with participants to develop social networks and engage in community recreation.

Providing side-by-side support and coaching to assist with development independent living skills.

Supporting successful tenancy and eviction prevention.
Establishing and/or maintaining personal wellness routines with participants.

Assisting participants with menu planning, grocery shopping, meal preparation, and cooking.

Assisting participants with developing and maintaining laundry routines.

Assisting participants with maintaining safety and cleanliness of living space.

Teaching money-management skills and assisting participants with creating a budget.
Collaborating with participants and the Supportive Housing Team to find meaningful daytime activities including employment and/or volunteer opportunities.

Assisting as liaison between tenant and landlord.
