SUMMARY OF THE POSITION



As the Housing Support Coordinator, your primary responsibility will include providing support coordination, individualized planning, supportive counselling, crisis support and overall assistance to reach recovery goals of individuals who experience mental health and substance use issues. You will support individuals housed in independent units as well as individuals living in a group home. You will be working as part of a team, and in collaboration with community partners.

HOURS OF WORK



The HSC will work thirty-five (35) hours per week, with the hours being flexible to the needs of clients. This position includes staff on-call responsibilities on a rotational basis.

SALARY RANGE:

$30.39 - $35.97

ESSENTIAL QUALIFICATIONS:



Completed Post-secondary degree/diploma in human services/human sciences or related field. Minimum three (3) years' experience within the community mental health field working with adults recovering from serious mental illness, substance use issues or supportive housing. Demonstrated ability of being a highly effective communicator with strong interpersonal and collaboration skills Experience in supporting individuals experiencing acute crisis and where possible taking action that is consistent with the clients crisis plan of action Prioritizing and organizing work is second nature and able to make recommendations Sound knowledge of community resources and the ability to liaise with community agencies, generic services, mental health professionals, specialized health care, recreation and leisure opportunities and vocational programs Ability to advocate and liaise with housing providers, landlords, boarding homes, shelter operations and operators of other housing facilities Proficient in the use of Word, Excel, electronic record keeping and data collection systems as well as CRMS Strong assessment skills and demonstrated ability to assess complex situations and make appropriate decisions independently Certification in First Aid/CPR, Non-Violent Crisis Intervention and suicide risk assessment preferred Must have a valid Ontario G driver's license, relevant insurance and use of a reliable vehicle Able to successfully complete and provide a current Police Vulnerable Sector Check
