Job Description

ICMH provides person centered, recovery oriented, and psycho-social rehabilitation (PSR) informed-care across a spectrum of services including housing and housing supports, group and one-on-one programming, and in Licensed Care settings.

We promote personal wellness and community integration by providing meaningful and effective services to our clients, residents and community members living with the experience of mental health challenges.

Scope and Level Definition:



Within the context of a person centered care model, the Recovery Coach provides a range of services that support individuals living with mental health and substance use issues. The Recovery Coach, as a member of an integrated, collaborative team, works to increase client independence through recovery-oriented activities and skill enhancement and development. This includes delivering person-centered support services and programs, assisting clients to maintain housing and support in daily living activities.

Duties and Responsibilities:



Provides individualized, person centered support to clients. Prepares, documents, monitors and evaluates personalized recovery plans with each client;

Visits clients in his/her apartment or in the community; observes client, identifies and assesses potential problem situations.

Assesses clients' functioning levels, identifying areas where individuals could benefit from assistance, teaching or guidance; ongoing monitoring of symptoms and behaviors.

Supports client's life skill needs, including cooking, planning menus and shopping lists, use of leisure time. Accompanies clients to appointments, shopping and/or leisure activities, as necessary. Assists clients to set goals that will enable them to move to a more independent living arrangement.

Consults with appropriate resources (eg., supervisor, activity program worker, case manager) as needed, according to clients' progress or concerns.

Arranges and plans, with clients, activities as required to involve and integrate in the community. Encourages the development of social events for clients on a small group basis. Helps clients to socialize outside of the home, to create support groups through leisure activities.

Assists clients to find both volunteer and paid work primarily through referral to appropriate programs within ICMH or the community.

Completes monthly reports and statistics documents; manages petty cash.

Attends staff meetings.

Performs other related duties as assigned.

Qualifications:



Education and Training:



A Bachelor's degree in Social Work or related field

Minimum of 2 years' experience working with the Psycho-Social Rehabilitation model and/or residential housing.

Minimum of 2 years' experience working with individuals with mental illness and substance use issues.

Required Certifications:



Standard First Aid and CPR

Violence Prevention

Clear criminal record check (Vulnerable Adults - Solicitors General's Office)

Drivers' abstract

Valid drivers' license and access to a reliable vehicle with a minimum $5 million liability 'business class' insurance

Preferred Certifications:



Motivational Interviewing

Applied Suicide Intervention Skills Training (ASIST)

Nonviolent Crisis Intervention

Harm Reduction

PSR Practitioner

Abilities and Skills:



Knowledge of Psychosocial Rehabilitation principles

Working knowledge of the Residential Tenancy Act

Working knowledge of harm reduction

Knowledge of community based resources and government agencies

Practical knowledge of best practices, the Residential Tenancy Act, ICMH Tenancy Policies, and all other relevant legislation

Strong problem solving skills

Knowledge and understanding of diverse populations

Excellent oral and written communication skills; second language an asset

Strong computer literacy; proficiency in Microsoft Office, case management program and internet

Ability to work both independently and within a team context

Physical ability to work effectively in an office environment as well as external setting

Job Types: Part-time, Permanent

Pay: $31.56-$33.28 per hour

Expected hours: 27.5 per week
