POSITION SUMMARY:



The Continuing Care Recovery Coach at Cedars Recovery provides crucial support to patients beyond their primary treatment stay, under the guidance of the Manager of Continuing Care. As a therapeutic guide, the Recovery Coach accompanies patients on their recovery journey, offering personalized support, goal setting, and fostering trusting relationships. This role ensures a seamless continuum of care by developing individualized care plans, connecting patients with community resources, and maintaining thorough documentation of progress. The ideal candidate possesses education in a related field, previous experience in addiction recovery or mental health settings, strong interpersonal skills, and a compassionate approach to patient care. SPECIFIC DUTIES & RESPONSIBILITIES:

In-Patient Support:



Establish and maintain rapport with patients during their in-patient treatment, fostering a supportive and trusting environment. Aid patients in setting up necessary tools, such as Zoom and Cedars Trails, to facilitate their engagement with post-treatment resources. Assist patients in accessing Second Stage Housing and connecting with alumni networks to support their transition post-treatment. Document all interactions with patients in the Cedars Information Management System (CIMS) and promptly communicate engagements to their primary counselor.

Outpatient Support:



Conduct outreach to patients at scheduled intervals to acknowledge and celebrate treatment milestones, providing guidance, referrals, and support upon request. Follow Case Management structure for connection points, check-ins, assessments, reviews, and other tasks for twelve months post-treatment. Attend and promote Cedars Alumni events as appropriate. Develop strong rapport with all Cedars Alumni. Provide appropriate referrals (both internal and external) as needed by the patient. Transition patients to Alumni Services in preparation of reaching their one-year milestone. Occasional travel may be required for Alumni engagement purposes.

Aftercare Processing Groups:



Facilitate both in-person and virtual Cedars Groups for patients transitioning to aftercare, ensuring attendance is accurately tracked using the online system. Document each patient encounter in CIMS and promptly communicate any patient concerns to the Continuing Care (CC) Manager and primary counselor. Compile and communicate attendance lists for in-person groups to ensure patients are informed and engaged.

Administrative Tasks:



Maintain thorough and accurate records of all interactions with patients using CIMS and tracking sheets. Regularly update and distribute the active Alumni List to facilitate ongoing communication and engagement. Participate in bi-weekly Continuing Care Case Conferencing sessions and engage in team-building activities to enhance collaboration and effectiveness. Perform any other related duties assigned by Cedars Management and Leadership to support the organization's overall objectives.

QUALIFICATIONS & CONDITIONS OF EMPLOYMENT:



Recovery Coaching or Mental Health and Addictions Diploma preferred. A comparable combination of education and experience may be considered. Proven experience in case management, particularly with individuals who are chemically dependent or have other process / concurrent disorders. Familiarity and comfort with Microsoft Office products, including Teams, Excel, and Word. Demonstrated proficiency in performing data entry and utilizing online data management systems. Experience facilitating group therapy sessions is highly desirable. Strong interpersonal skills and the ability to build rapport with individuals in various stages of recovery. Excellent communication skills, both verbal and written, with the capacity to convey information effectively and empathetically. Ability to work collaboratively within a multidisciplinary team environment, demonstrating flexibility and adaptability. Strong organizational skills with the ability to prioritize tasks and manage time effectively. Commitment to ongoing professional development and willingness to engage in training opportunities as required. Ability to maintain confidentiality and adhere to ethical standards in all interactions with patients and colleagues. Reports directly to the Continuing Care Manager. Three-month probationary period and annual performance review. Criminal Record with Vulnerable Sector Check required. Statutory holidays as per the BC Employment Standards Act (BCESA). Will have and maintain current First Aid and CPR certification. Training in CIWA Protocol assessment / Naloxone Administration / Seizure Protocol. * Employees who identify as being in recovery will need to be engaged in a program of recovery and substance free for a minimum of two (2) years.
