Coordinator
to help us connect with more individuals in need.
Position Summary:
The Housing Coordinator is responsible for building and maintaining strong referral pathways into our housing programs. This role involves engaging with hospitals, shelters, community agencies, and social service providers to identify and qualify potential residents. The Coordinator also works directly with new and prospective clients to complete intake applications and assists in obtaining additional supports such as housing subsidies or social benefits.
Key Responsibilities:
Build and maintain referral relationships with hospitals, shelters, mental health and addiction services, and homelessness prevention organizations
Conduct outreach visits to community partners to promote awareness of our housing programs and intake process
Identify and connect with prospective clients and assist them in completing intake applications for available housing units across all of our managed facilities in St Thomas, London and Strathroy
Collect required documentation and liaise with applicants to ensure all eligibility requirements are met
Coordinate with internal staff to support smooth transitions into housing for approved residents
Seek out and apply for available housing or support subsidies for both new and existing residents (e.g., through city programs, provincial supports, or charitable organizations)
Attend community meetings, case conferences, and networking events to maintain a visible presence within the local housing and social services ecosystem
Maintain accurate, confidential records and data tracking of referrals, intakes, and outcomes
Collaborate with the internal team to help develop efficient intake procedures and identify service gaps or opportunities
Work with funding organizations to apply for more grants and subsidies that can fund the clients and/or operations of the facilities
Qualifications:
Post-secondary education in Social Work, Community Services, Addictions & Mental Health, or a related field
Minimum 2 years of experience in outreach, housing services, or community engagement roles
Strong understanding of local homelessness support systems, shelters, and housing subsidy programs in London, Ontario
Excellent communication, relationship-building, and organizational skills
Comfortable with fieldwork, community visits, and building rapport with diverse populations
Ability to work independently and manage multiple leads and referrals simultaneously
Valid driver's license and access to reliable transportation required
Vulnerable Sector Police Check required

Pay: $21.00-$26.00 per hour
