Position Purpose Summary:
The Canadian Government has created Reaching Home. Canada's Homelessness Strategy is a community-based program aimed at preventing and reducing homelessness across Canada. Reaching Home supports the goals of the National Housing Strategy to support the most vulnerable Canadians in maintaining safe, stable, and affordable housing and to reduce chronic homelessness nationally. The Salvation Army is a non-profit organization that provides the Housing With Supports Program through funding of the Canadian Government.
Housing With Supports is a program that provides both rent subsidies, as well as the assignment of a Housing Coordinator who provides Intensive Case Management and connections to community supports over the duration of a year or if needed. This is a goal orientated program, and the main objective is financial independence and personal stability for clients involved.
Accountabilities:Provide intensive case management support for those housed through Salvation Army Housing With Supports Program, which includes, but is not limited to: integration of previously homeless persons into the community, client centered goal setting displaying increase client independence, secure permanent income source, model necessary life skills, obtain ID's and make referrals to various support services. Complete necessary paperwork such as assessments follow up interviews, monthly statistical reports, data entry on a computerized program, filing, etc. Must attend bi-weekly case conferencing meetings and submit all required statistical information Work closely with the Landlord Support Liaison Meet with Supervisor regularly to give and receive updates Attend Housing With Supports and In-House meetings as required Develop strong relationships with landlords and develop extensive knowledge of the RTA (Residential Tenancies Act) Adhere to all Housing With Supports and Salvation Army guidelines Advocate for Individuals and the homeless population in a professional manner Entering client information into HIFIS in a timely manner Must do home visits Must transport clients

CRITICAL RELATIONSHIP MANAGEMENT
Governance Boards and Councils:
Internal:Maintain communication with team lead Maintain communication with caseload individuals

External:Maintain communication with community funders and other agencies or community supports

MANAGERIAL/TECHNICAL LEADERSHIP RESPONSIBILITY:Reports directly to: Team Lead

FINANCIAL AND MATERIALS MANAGEMENT:Responsible for ensuring individuals on caseload receive basic necessities through programs and services (i.e., food hampers, bus tickets, etc.)

WORKING CONDITIONS:Working environment is typically in the office in generally agreeable conditions. Working environment requires travelling with individuals on caseload and entering individuals' residence - potential for high-risk situations. These visits are done alone unless otherwise determined by supervisor

Education, Qualifications and Certifications:Must possess a related certificate or diploma in Human services.

Experience and Skilled Knowledge RequirementsWorking in a social services environment/sector or with the homeless population an asset

Skills and Capabilities:Must possess knowledge of the Housing With Supports Program Must provide an acceptable Criminal Record with Vulnerable Sector check upon hire. Must be open to attending work related training, events, and courses. Must possess strong computer, organizational and time management skills. Must understand, and compassion for, issues surrounding homelessness, such as mental health and addiction. Up to Date Standard First Aid Certificate Required. Have an extensive knowledge of working alone safety practices or a "Working Alone Training" course is necessary It is essential to own a vehicle that can be used daily, and provide a copy of driver's license, abstract and proof of insurance Must possess strong verbal and written skills

This is a fixed term position to August 31, 2026. Hours are 40 hrs/week. Target pay is $32.00 per hour.
Compensation:
The target hiring range for this position is $25.29 to $31.62 with a maximum of $37.94.
Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.
