We are seeking compassionate and dedicated Home Support Workers to provide exceptional in-home and community-based care to individuals with diverse needs. This role involves assisting clients with daily activities, health monitoring, and emotional support, ensuring they maintain independence and quality of life. The ideal candidate will have experience in senior care, assisted living, or working with individuals with disabilities, including those with dementia or developmental disabilities. A strong understanding of patient safety, HIPAA compliance, and caregiving best practices is essential for success in this role.
Responsibilities
Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
Provide medication administration and monitor vital signs in accordance with care plans
Support clients with meal preparation, cooking, and nutritional needs while adhering to dietary restrictions
Perform patient observation to identify changes in condition or behavior and report findings to healthcare professionals
Utilize assistive devices such as Hoyer lifts and manage heavy lifting safely to ensure client mobility and safety
Maintain accurate documentation of care provided, including patient monitoring and adherence to HIPAA regulations
Support clients with memory care needs, including Alzheimer's and dementia care strategies
Engage in social activities and provide companionship to promote mental well-being
Follow established care plans and protocols for hospice, home health, or long-term care settings
Ensure a clean and safe environment by performing light housekeeping tasks as needed
Experience
Prior experience in home & community care, assisted living, nursing homes, or hospice care preferred
Knowledge of working with individuals with developmental disabilities, autism, or mental health conditions such as dementia or Alzheimer's disease
Certification in First Aid and CPR required; additional certifications such as HIPAA training are a plus
Experience with patient monitoring, medication administration, and vital signs measurement highly desirable
Familiarity with assistive devices like Hoyer lifts and techniques for heavy lifting is advantageous
Strong communication skills for working effectively with clients, families, and healthcare teams
Background in social work or nursing is beneficial but not mandatory; a caring attitude and willingness to learn are essential
