We are seeking compassionate and dedicated Home Support Workers to provide essential care and assistance to individuals in their homes and community settings. Duties
Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
Provide medication administration and monitor vital signs in accordance with care plans
Support patients with memory care needs such as Alzheimer's or dementia through specialized techniques
Prepare nutritious meals tailored to dietary requirements and assist with feeding when necessary
Perform light housekeeping tasks including cleaning and laundry to maintain a safe and comfortable environment
Utilize equipment such as Hoyer lifts or other assistive devices for safe patient transfer and mobility assistance
Observe and document patient condition, noting any changes in health status or behavior for reporting to healthcare professionals
Follow HIPAA guidelines to ensure patient confidentiality and privacy at all times
Support patients during social activities and encourage engagement to promote mental well-being
Collaborate with healthcare teams to implement individualized care plans effectively
Skills
Proven experience in home & community care, assisted living, nursing homes, or long-term care environments
Knowledge of HIPAA regulations and patient privacy standards
Certifications such as CPR and First Aid are highly desirable
Experience working with individuals with developmental disabilities, autism, or special needs including Alzheimer's or dementia care
Ability to monitor vital signs and recognize signs of medical emergencies
Strong caregiving skills with patience, compassion, and excellent communication abilities
Physical capability for heavy lifting and assisting with mobility aids like Hoyer lifts or resident transfers
Familiarity with medication administration protocols and care planning processes
Ability to work independently while following detailed care instructions
Prior experience working within hospice care or home health settings is advantageous
