We are seeking compassionate and dedicated Home Support Workers to provide in-home and community-based care to individuals with diverse needs. Duties
Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
Provide medication administration and monitor vital signs in accordance with care plans
Support clients with memory care needs, including Alzheimer's and dementia care
Prepare nutritious meals tailored to dietary requirements and assist with feeding as needed
Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment
Utilize specialized equipment such as Hoyer lifts for safe patient transfers and mobility assistance
Observe and document patient condition, behavior, and progress while adhering to HIPAA regulations
Implement individual care plans developed by healthcare professionals or social workers
Offer emotional support and companionship to promote mental well-being
Collaborate with healthcare teams to ensure comprehensive patient care
Experience
Candidates should have prior experience in home & community care, assisted living, nursing homes, or long-term care environments. Additional preferred skills include:
First aid and CPR certification
Experience working with people with developmental disabilities, autism, or disabilities requiring specialized support
Knowledge of Alzheimer's disease, dementia, memory care, hospice care, and resident safety protocols
Familiarity with medication management and patient monitoring techniques
Experience working within HIPAA guidelines ensuring client confidentiality
Ability to perform heavy lifting safely when required (e.g., resident transfers)
Strong observational skills for patient safety and health status updates
Background in social work or nursing is advantageous for understanding comprehensive care planning
This role is vital in supporting individuals' independence while ensuring their safety and comfort through compassionate caregiving.
