Overview

We are seeking compassionate and dedicated Home Support Workers to provide in-home and community-based care for individuals with diverse needs. This role is vital in enhancing the quality of life for clients by assisting with daily activities, health monitoring, and emotional support. Duties

Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support Provide medication administration and monitor vital signs in accordance with care plans Support clients with meal preparation, feeding, and nutritional needs Perform light housekeeping tasks including cleaning and laundry to maintain a safe and comfortable environment Use specialized equipment such as Hoyer lifts to assist with patient transfers safely Observe and document patient condition, report changes to healthcare professionals, and update care plans accordingly Offer companionship and emotional support to reduce feelings of isolation or distress Support clients with dementia or Alzheimer's care, memory care, or behavioral challenges Follow HIPAA guidelines to ensure confidentiality and privacy of client information Assist with transportation to medical appointments or community activities as needed

Qualifications

Prior experience in home health, assisted living, nursing home, hospice care, or working with individuals with developmental disabilities or autism is preferred Certifications such as First Aid, CPR, and HIPAA compliance are highly desirable Knowledge of medication administration, vital signs monitoring, and dementia or Alzheimer's care is advantageous Ability to perform heavy lifting safely when required and use assistive devices like Hoyer lifts Strong observational skills for patient monitoring and safety awareness Compassionate attitude with excellent communication skills and patience when working with diverse populations Experience working within care plans and understanding of social work principles is a plus Valid driver's license may be required for transportation duties
