About the Opportunity
Reporting to Coordinator, the Home Share Manager support and equip Home Share Providers (HSP) to ensure person-centered services and opportunities for people supported to reach the best life of their choosing. The Home Share Manager (HSM) will act as a liaison between HSP, enVision Community Living, and other stakeholders; and be a resource person HSP in terms of policies, procedures, and initiatives. Other stakeholders include but are not limited to families of person supported, Community Living and Disability Services (CSW), Residential Care Licensing, and community members.
As a
Home Share Manager
, you would:Act as the subject manager expert of the organization for Home Share Program (HSP).
Promote equality, inclusion, community participation, relationship development, and employment to effectively facilitate opportunities for valued roles and good life experiences.
Gather relevant and valid information from a variety of sources to gain an understanding of the needs, skills, goals and interests of persons supported.
Be involved in developing, implementing and evaluating service delivery. Use formal and informal assessments as required.
Promote equality, inclusion, community participation, relationship development, and employment to effectively facilitate opportunities for valued roles and good life experiences.
Ensure HSPs are equipped to provide quality support. Identify and provide resources that would benefit HSP in fulfilling their roles. Maintain regular contact with HSP to coach and lead them.
Ensure completion and secure storage of records and documentation in accordance with enVision's policies and procedures, and provincial requirements.
Provide person-centered supports to people residing in the community.
Accomplish this by presenting a holistic approach, helping people to engage in activities that they enjoy.
Encourage building connections and relationships,
Promote and encourage learning and development in their daily living, building life skills.
Encourages people supported to participate in the preparation of their own meals and snacks, doing laundry, engaging in home maintenance, making financial decisions, and all other tasks necessary to living in a home in community.
Administer medications accurately to individuals supported, as assigned. Monitor and follow- up on basic health and medical needs.
Qualifications
Completed Supervision in Human Service Settings (Disability and Community Support) or willing to obtain Educational Background in Disability Management, Human Services, Business Management or any related field. A combination of work experience will also be considered.
Minimum two years of supervisory/management experience preferred
Proficiency in Microsoft Office Suite 365 and Database Platform
Excellent organizational skills, with the ability to attend crisis management
Knowledge and experience in Case Management
Ability to interpret policy & procedure and translate to day-to-day operations
Demonstrated ability to advocate and promote self-advocacy
Excellent written and oral communication skills and ability to engage with diversified population
Be caring, compassionate, patient, respectful, and understanding.
Practice privacy and confidentiality
Ability to take the initiative, solve problems and work independently
Ability to connect with community resources
Experience working in the Disability Sector preferred
Experience in Non-Profit is an asset
Additional Requirements
Valid Manitoba Class 5F (Full) License and access to vehicle required.
Valid First Aid & CPR Level A or C certificate or willingness to obtain.
Criminal record check with Clear Vulnerable Sector check (current within last 3 months).
Clear Child and Adult abuse registry check (current within last 3 months).
Working Conditions:
Work 20 Hours per week as a Home Share Manager (Monday to Friday between 8:00am to 5:00pm)
Work 20 Hours per week as a Direct Support Professional (Schedule may include days, evening, afternoon, weekend, and overnight)
Working Environment: 100% On-site.
Interact with family members, staff, visitors, community members, and government agencies.
Travel to-and-from supported person's homes, and within the community.
Ability to lift/push/pull up to fifty (50) lbs (sometimes more), dependent upon location.
Manual dexterity required to use desktop/laptop computers, or other devices (i.e. tablets).
