We are seeking compassionate and dedicated Home Care Workers to provide exceptional in-home care and support to clients with diverse needs. This role involves assisting individuals with daily activities, ensuring their safety, and promoting independence within their homes and communities. The ideal candidate will have a strong background in patient care, caregiving, and long-term care, with a focus on enhancing quality of life for those in need of assistance. Responsibilities
Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
Provide medication administration and monitor vital signs in accordance with care plans
Support clients with meal preparation, feeding, and nutritional needs while adhering to dietary restrictions
Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment
Observe and document patient conditions, report changes to healthcare professionals promptly
Utilize specialized equipment such as Hoyer lifts and assist with transfers safely
Support clients with cognitive impairments including dementia or Alzheimer's disease through memory care techniques
Implement individualized care plans tailored to each client's needs and preferences
Maintain HIPAA compliance by safeguarding client confidentiality at all times
Provide emotional support and companionship to promote mental well-being
Follow all safety protocols including CPR and first aid procedures when necessary
Experience
Candidates should possess relevant experience in home & community care settings, assisted living facilities, nursing homes, or group homes. Prior work with individuals who have developmental disabilities, autism spectrum disorder, or dementia is highly desirable. Experience with hospice or palliative care is a plus. A background in nursing or social work can enhance effectiveness in this role. Certifications such as CPR and first aid are preferred. Candidates must demonstrate patience, excellent observational skills for patient monitoring, the ability to perform heavy lifting when required, and familiarity with HIPAA regulations. Previous experience working with residents' care plans and medication administration is essential for success in this position.
