We are seeking compassionate and dedicated Home Care Workers to provide exceptional in-home support to clients in need of assistance with daily activities. This role offers an opportunity to make a meaningful difference in the lives of individuals requiring personal care, companionship, and health monitoring. The ideal candidate will possess a strong background in senior care, disability support, or home health services, with a focus on promoting independence and well-being within clients' homes and communities.
Responsibilities
Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
Provide medication reminders and assist with medication administration following care plans
Monitor and record vital signs such as blood pressure, pulse, and temperature
Support patients with cognitive impairments such as dementia or Alzheimer's through memory care techniques
Prepare nutritious meals tailored to dietary needs and assist with meal planning and preparation
Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment
Utilize specialized equipment such as lifts or other mobility aids to ensure safe transfers and positioning
Observe and report changes in health status or behavior to office staff or family members
Experience
Prior experience in home & community care, assisted living, nursing homes, hospice care, or long-term care settings is an asset but not required. Training can be provided. Experience working with people with disabilities, dementia, Alzheimer's disease, or autism is highly valued. Knowledge of patient monitoring, medication administration, care planning, and HIPAA compliance is essential. Additional skills that enhance suitability for this role include CPR certification, first aid training, group home experience, patient observation skills, heavy lifting capability, and familiarity with hospice or memory care environments. A background in social work or nursing is beneficial for understanding comprehensive patient needs. Prior experience working with diverse populations requiring specialized support--such as those with developmental disabilities or chronic health conditions--is preferred.
