As a Care Assistant with RISE Care, you will play a key role in delivering this approach in clients' homes. You'll work one-on-one with clients to provide personal care, companionship, and restorative support that helps them remain active, engaged, and independent. You will work closely with our care team, contributing observations, ideas, and encouragement that support each client's goals.

Key Responsibilities



1. Personal & Supportive Care



Assist clients with activities of daily living (bathing, dressing, grooming, toileting, mobility, and transfers) as outlined in care plans. Support safe ambulation using mobility aids when required. Encourage independence while maintaining comfort, safety, and dignity.

2. Restorative & Wellness Support



Support clients with their

restorative care plans

, including light movement, strength, balance, and functional activities (within training and scope). Encourage healthy routines, confidence, and consistency with daily activities. Motivate and positively reinforce client progress toward independence and functional goals. Observe barriers to participation and creatively adapt activities to suit client abilities and preferences.

3. Health Observation & Reporting (as delegated)



Observe and report changes in physical condition, mood, cognition, appetite, or safety. Measure and record vital signs when delegated by an RN/LPN. Accurately document care provided and communicate concerns promptly to the care team.

4. Nutrition & Hydration Support



Assist with meal preparation and hydration as outlined in care plans. Encourage adequate fluid and nutritional intake while respecting dietary needs.

5. Home Safety & Environment



Maintain a clean, safe, and organized care environment. Perform light housekeeping related to client care (laundry, dishes, linens, sanitation). Identify and report safety concerns or hazards in the home.

6. Emotional, Social & Companionship Support



Provide companionship and meaningful conversation. Support social engagement, routines, hobbies, and interests. Build respectful, trusting relationships with clients and families. Respect privacy, cultural values, and personal choices at all times.

7. Medication Assistance (if trained and delegated)



Provide medication reminders or assist with self-administration as outlined in the care plan. Observe and report any concerns or side effects to the supervising nurse.

Supervision & Accountability



Care Assistants perform only tasks within their training and demonstrated competence. Delegated tasks are overseen by an RN/LPN, who retains responsibility for assessment and supervision. Any uncertainty, safety concern, or change in client condition must be reported immediately.
Qualifications & Attributes

Required / Preferred:



Health Care Aide (HCA) certificate preferred (or willingness to train) Current CPR & First Aid Criminal Record Check with Vulnerable Sector Reliable transportation (asset)
