Qualifications:

Post-secondary education in Social Services or a related field 1-3 years of experience working with newcomers providing frontline services and outreach Strong analytical and decision-making skills, particularly in crisis intervention Excellent communication skills in English; fluency in a relevant second language is advantageous Outstanding organizational, coordination, and interpersonal skills Sensitivity to social and multicultural dynamics Proficiency in MS Office, Teams, Outlook Strong interpersonal and communication skills to foster trust and clarity with clients, colleagues, and external partners Collaborative mindset to work effectively within a multidisciplinary team Client-centered approach that prioritizes the needs, dignity, and autonomy of individuals and families Cultural humility and sensitivity to engage respectfully with diverse communities Adaptability and problem-solving skills to respond to evolving client needs Commitment to shared goals through active participation in case conferences, team meetings, and outreach initiatives Professional integrity and accountability in maintaining records and ensuring continuity of care Proactive engagement with service partners to co-create outreach strategies and improve access to supports

DUTIES & RESPONSIBILITIES



Provide intake assessment, understanding clients' healthcare needs and social determinants of health Work with individuals and their families to access the most appropriate services to suit their needs and circumstances Identify and address barriers to accessing healthcare services and community resources. Maintain accurate and detailed client records, progress notes, and documentation in compliance with privacy regulations. Prepare comprehensive reports on client demographics, service utilization, outcomes, and challenges encountered. Monitor and evaluate client progress, adjusting care plans as needed to achieve desired outcomes Provide information eligibility criteria and facilitate navigation health and social support resources Maintain up to date information on community services and opportunities and liaise closely with service providers to ensure optimum delivery of services. Provide information and guidance on available healthcare services, programs, and wrap-around support services. Assist clients in navigating the healthcare system, social support system, newcomer support system and support in clients accessing appropriate resources and programs. Coordinate referrals to service providers, ensuring continuity of care. Collaborate with healthcare professionals, social workers, and community agencies to facilitate coordinated care. Participate in case conferences, multidisciplinary team meetings, and community outreach activities. Advocate for clients' rights and needs within the healthcare system and community services. Provide emotional support, education, and empowerment to clients Collaborate with community partners to address social determinants of health impacting clients. Participate in network of Neighbourhood-based services providers

Immunization Requirement

- Proof of immunization (MMR, Varicella, TB, Hepatitis B) and a valid mask fit test are required prior to starting, in accordance with Ontario Occupational Health and Safety Act and Hospital protocols.

Screening

- As a condition of employment, the successful applicant must complete a Vulnerable Sector Screening.

Job Type:

A full-time contract position for 1 year.

Work Location(s):

Various locations within TNO/HATP (45 Overlea Blvd) FHC (10 Gateway Blvd) Health Access Taylor Massey (4 The Mkt Pl, East York, ON M4C 5M1), MGH (825 Coxwell Ave)

Schedule:

35 hours per week, with working hours between Monday to Saturday 8 am to 8 pm (evenings and weekends may be required)
