Hours of work:
35 hours/week -7:30 am to 3:30 pm Monday through Friday (ability to work flexible hours to either accommodate early morning or evening appointments).
Why This Role Matters
The Tuberculosis team in the Communicable Diseases Division is seeking a motivated, collaborative, and self-directed professional. This role is essential to delivering outreach services, engaging with residents, and fostering long-term community health capacity in the ongoing effort to prevent and manage tuberculosis.
What you will do:
As an integral member of the Tuberculosis team, you will be involved in supporting individuals and families affected by tuberculosis, including the delivery of Directly Observed Therapy (DOT)
Visit clients to deliver medication and to observe medication ingestion, assess for side effects
Promote adherence to TB treatment and help the client understand TB
DOT activities also include identifying social, cultural and economic barriers that may prevent clients from completing the treatment plan
You will liaise with the Public Health Nurse (PHN) to provide and receive updates on clients' progress, medication side effects and non-adherence issues with treatment program and/or clarification of medical information
Facilitating referrals to appropriate community resources
Liaise with physician offices regarding clients' appointments and status of progress reports as well as potentially accompany clients to their TB medical appointments or participate in discharge planning and case conferences
You will liaise with community partners/agencies. You will also work on program planning, evaluation and continuous improvement projects
Work with Peel's diverse community, including working with vulnerable and marginalized individuals

What You bring:
Completion of post-secondary education in a health or social services field, or a combination of post-secondary education in another area and relevant work experience
A minimum three years related experience, including client advocacy work and networking with community partners
Knowledge of tuberculosis and communicable diseases
Knowledge of individual/family dynamics, community resources and supportive counseling
Knowledge of outreach principles
Attention to detail
Valid G class license and access to a reliable vehicle
Fluency in additional languages is an asset

An ability to assess/communicate and document findings from home and telephone visits
Strong verbal and written communication skills
Strong technological skills with experience using videoconferencing platforms such as OTN/Teams
Demonstrated problem solving, critical thinking and decision-making skills

Ability to work independently and within an interdisciplinary team environment
Ability to multitask and prioritize

Work Mode & Job Location:
You will be working in the community and reporting into 7120 Hurontario Street, Mississauga, ON.
This position requires work in office and within the community. Staff are required to be in the office a
minimum of 2 days/week
and should expect to be in the community for 5 days/week as needed to fulfill duties such as home visits, medication deliveries, and transporting specimens.
It's important to note that your off-site work location must be within the province of Ontario. This ensures that we're compliant with all regulations and policies.
