Overview Queer Yukon Society is looking to hire a full-time Health Navigator to support 2SLGBTQIA+ individuals and families to make informed choices about their healthcare, including working with them to identify their needs, providing information about care options, and addressing concerns. The Health Navigator should be able to understand and interpret medical information and have experience supporting clients with challenging decisions as well as health system navigation. This will be a full-time position for a support worker, an outreach worker, a peer worker, social worker, or someone with equivalent lived experience. Duration of contract: one year renewable contract Location: Whitehorse preferred, remote work from other Yukon communities will be considered. Must live in the Yukon. Hours: 35 hours per week Wage: $38-$40/hour based on experience and location Benefits: Organizational benefits plan includes coverage for: prescriptions, dental care, paramedical (e.g. counseling, massage, physiotherapy, acupuncture, chiropractors, etc.), vision care, life insurance, and disability insurance. Equipment: Employees will be provided necessary equipment to complete tasks. A laptop & phone will be provided. Responsibilities include:
  • Support 2SLGBTQIA+ individuals and families to make informed choices about their healthcare, including working with them to identify their needs, providing information about care options, and addressing concerns.
  • Identify client needs, provide information about care options, and address concerns.
  • Advance clients\xe2\x80\x99 safety, agency and informed consent in their healthcare decisions, accompanying 2SLGBTQIA+ individuals to appointments or otherwise advocating for their needs as requested.
  • Coordinate scheduling and administrative tasks of the Health Navigation Team, including management of the Jane scheduling platform.
  • Communicate with healthcare providers and Government of Yukon to identify pathways and barriers to care.
  • Stay up-to-date on best practices in 2SLGBTQIA+ healthcare as well as services available to Yukoners.
  • Work with Healthcare Navigation - clinical role to support clients with booking appointments, gathering documentation and navigating Insured Health Services, Non-Insured Health Benefits, gender marker changes and other relevant systems.
  • Develop public health education resources and resources for system navigation for 2SLGBTQIA+ Yukoners in collaboration with Queer Yukon\xe2\x80\x99s Health Navigator - Clinical, the Health & Education Manager, and other QYS staff, and work collaboratively to identify areas where training should be provided to healthcare providers or the community at large.
  • Demonstrate conflict management skills
  • Adapt role and activities to meet Regional needs in addressing policy challenges and changes
  • Follow all policies, procedures, and guidelines as set by Queer Yukon Society
  • Participate in relevant staff and team meetings,
  • Keep your workspace(s) tidy and well organized,
  • Ensure all client information is kept confidential and is securely stored, and
  • Other duties as mutually agreed upon, according to capacity.
The ideal candidate will have:
  • A strong knowledge of or first-hand experience with the 2SLGBTQIA+ community
  • Planning, priority setting, time management, organization and coordination skills.
  • Excellent communication skills, particularly with vulnerable populations.
  • In depth experience, knowledge, and/or training in Trans related healthcare
  • Prior experience working in the healthcare sector, and/or systems navigation
  • Anti-racist and/or any other anti-oppression training or experience
  • A strong knowledge or experience with Yukon\xe2\x80\x99s First Nations communities, and
  • Basic familiarity with Gmail, Google Drive, Zoom.
  • Lived experience of barriers to medical care (including transition-related barriers, medical racism, and/or experience in a remote community) will be considered an asset.
  • Valid driver\xe2\x80\x99s license and access to a vehicle, or other reliable transportation to attend medical appointments will be considered an asset.
  • Experience working in the non-profit sector is an asset.
  • CPR or BLS certification or willingness to complete within 6 month of hiring
  • TDG certificate an asset or willingness to complete within 6 month of hiring
