Responsibilities
Coordinate and oversee daily care activities for residents or patients, including assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support.
Monitor patient health by observing vital signs, tracking changes in condition, and reporting findings accurately while adhering to HIPAA regulations to ensure confidentiality.
Assist with medication administration following prescribed protocols and document all care provided meticulously.
Develop and implement individualized care plans in collaboration with healthcare professionals, ensuring each resident's needs are met effectively.
Support residents during meal preparation and feeding, ensuring nutritional needs are satisfied in a safe and respectful manner.
Perform light cleaning duties to maintain a hygienic environment that promotes health and safety for residents and staff alike.
Engage in patient observation for signs of dementia, Alzheimer's disease, autism spectrum disorder, or other developmental disabilities; provide appropriate support accordingly.
Conduct regular patient monitoring including vital signs checks and behavioral observations to ensure ongoing well-being.
Collaborate with social workers, nursing staff, and family members to update care plans and address evolving needs.
Participate in training sessions on CPR, first aid, hospice care principles, and other relevant certifications to enhance caregiving skills.
Maintain accurate documentation related to patient care activities while respecting privacy standards under HIPAA regulations.
Requirements
Proven experience working in assisted living facilities, nursing homes, home health agencies, or group homes with a focus on senior or disability care.
Knowledge of home & community care practices including assisting with ADLs and patient monitoring techniques.
Certification in CPR and first aid is required; additional certifications such as HIPAA compliance or hospice care are advantageous.
Demonstrated ability to work compassionately with individuals with developmental disabilities, dementia, Alzheimer's disease or autism spectrum disorder.
Strong communication skills for effective collaboration with healthcare teams and families.
Familiarity with medication administration protocols and maintaining detailed care records.
Experience in cleaning routines necessary for maintaining sanitary living environments.
Ability to handle sensitive situations discreetly while providing empathetic support to residents' emotional needs.
