Overview


We are seeking a dedicated and compassionate Healthcare Coordinator to join our team here at Always Best Care. This role is essential in ensuring that our patients receive high-quality care and support in various settings, including assisted living facilities, nursing homes, and home care environments. The ideal candidate will have experience in senior care, hospice care, and working with individuals with disabilities. A strong focus on patient care and the ability to work collaboratively with healthcare professionals is crucial for success in this position.

Responsibilities



Coordinate patient care services across multiple settings, ensuring continuity and quality of care. Develop and implement individualized care plans tailored to the needs of each patient. Assist with daily living activities, including meal preparation, medication administration, and personal hygiene. Provide support for patients with dementia and other cognitive impairments. Facilitate communication between patients, families, and healthcare providers to address concerns and updates regarding patient health. Supervise caregiving staff to ensure compliance with care standards and protocols. Conduct regular assessments of patient needs and adjust care plans as necessary. Maintain accurate records of patient progress and changes in condition. Assist with heavy lifting when necessary to support patients in mobility.

Qualifications



Previous experience in assisted living, nursing home settings, or senior care is preferred. Knowledge of hospice care practices and working with individuals with disabilities is a plus. Familiarity with parkinsons care techniques and strategies for effective communication. Strong caregiving skills including cleaning, meal preparation, and personal assistance. Ability to perform physical tasks such as heavy lifting as required by patient needs. Excellent interpersonal skills with a compassionate approach to patient interactions. Strong organizational skills to manage multiple responsibilities effectively. Certification in nursing or relevant healthcare fields is advantageous but not mandatory. Job Types: Part-time, Permanent

Pay: Up to $20.00 per hour

Expected hours: 7 per week
