Are you able to lead a multidisciplinary team while supporting individuals facing homelessness and complex health needs? Use your leadership and clinical expertise to drive meaningful change in clients' lives as a Health & Wellness Manager.



Reporting to the Senior Director of Health and Wellness, the Health and Wellness Manager leads a multidisciplinary team that supports Mustard Seed clients experiencing multiple barriers, resulting in chronic homelessness and/or increased utilization of emergency health services, to pursue meaningful life changes and navigate health systems. The Health and Wellness Manager is responsible for building a team, which includes staff, volunteers, and student interns, who are responsible for assessing each client's mental, cognitive, and physical health and addiction needs, developing, and implementing a personal care plan to assist the client in moving forward and achieving their goals, and enabling appropriate system navigation and access.

MAJOR DUTIES AND RESPONSIBILITIES




Ensure that assessment of clients' health needs is completed and is followed by referrals to receive appropriate diagnosis and treatment; Ensure that clients receive direct care in relation to addiction, mental health, cognitive health, physical health and wellness issues; Ensure that ongoing assessment and support of clients is carried out; Ensure proper documentation of client interactions, client referrals, and client progress in AVA EMR, and ensure that applicable regulations (FOIPA/HIA) are followed; Facilitate emergency interventions in collaboration with other departments, as needed, Provide leadership and support to Health and Wellness staff, including team leads; Develop and maintain effective working relationships with external agencies; Provide collaboration and support to internal housing and support systems at The Mustard Seed; Ensure that tracking of statistics and preparation of reports is completed as required; Ensure staff are properly trained and orientated, review training material to ensure relevancy; Actively recruit third-party and practicum placements to fill appropriate positions within the Mustard Seed; Ensure spiritual engagement and provide guidance to the team;




MINIMUM QUALIFICATIONS AND SKILLS




Post-graduate degree in Psychology, Counselling, Social Work, or related health field; PhD preferred; Minimum five years experience working in Mental Health; Eligible to pursue registration with the appropriate governing college of applicable profession or will be registered within a year after starting the position; A passion to be part of ending homelessness and poverty in Alberta; Experience working with vulnerable populations; particularly, people with addictions and/or mental health impairments, and experiencing homelessness; Experience managing and leading staff; Excellent written, oral, and interpersonal skills and ability to effectively communicate with members of the multidisciplinary team and community agencies; Demonstrated knowledge of Edmonton's community (mental) health and addiction resources, and adept in navigating those systems; Knowledge of FOIP and HIA; A self-starter with the ability to work in a multidisciplinary team to accomplish goal; Proficient computer skills (Microsoft Office Programs); Experience with Homeless Management Information System (HMIS) would be considered an asset; Superior organizational, time management, and advocacy skills.

Time in excess of regular working hours may be required on occasion;
Traveling, driving a personal vehicle, sitting, and standing are required as part of this role; Office space will be provided; Moderate physical effort required to process documents and data; Moderate mental effort required to successfully accomplish the essential job functions such as responding to clients, intense situations and providing information; Very fast-paced and demanding work environment;
