ROLE PURPOSE
This position will provide support to residents of the Connective Support Society's programs in areas relating to individual risk and needs in order to ease their transition to the community and assist in their reintegration efforts, ensuring the orderly and safe operation of the facility, and maintaining the goals of the program. This position is responsible for the provision and coordination of services to residents in the community.
STATUS
This position is non-unionized. Open to applicants of all genders.
HOURS (40 HOURS PER WEEK)
Monday to Friday, 1200-2000
This is a hybrid role that is primarily outreach-based. There will be times that the worker will need to work at SHARP and therefore will need the ability to get reliability security clearance.
KEY ACCOUNTABILITIES
Works with the client, researches background material, and seeks the input of other professionals and family members, to assess needs and intervention possibilities
Works with the client to consider issues and agree on an intervention and support plan with measurable short and long-term goals
Ensures the client?s intervention and support plan addresses issues of daily living, including but not limited to housing, nutrition, clothing, hygiene, leisure, lifestyle, access, employment, and education
Interacts with other professionals, family members, and organizations to ensure services, admissions, and treatments are in the client?s interest and supportive of the long-term goals
EXPERIENCE AND SKILLS
A Diploma in Social Services and one-year relevant experience is preferred; inhouse training can be provided for the right candidate
Professional or lived experience working with vulnerable populations experiencing multiple barriers
A demonstrated and clear ability to respond well in crisis situations
REQUIREMENTS
Ability to successfully complete an Enhanced Security Clearance
Ability to successfully pass a reference check
Current Emergency First Aid Certificate
Valid class 5 driver?s license
Proof of Covid-19 Vaccination
Completed TB Screening
Permanent Residency or Canadian Citizenship

The starting wage is $31.12 per hour plus benefits.
