Join our Foundations team, where the role of the Foundations Mentor is to provide direct support to adults with intellectual and developmental disabilities as they transition from life as a student to life as an adult. As a Foundations Mentor, you will provide one-to-one and/or group support, advocacy, supportive problem-solving, systems navigation, connection to resources and supports, etc., based on the individualized goals of each person supported. This position provides services to Perth County and is based out of our Stratford office.
This current employment opportunity is a temporary full-time contract position from date of hire to March 31, 2026, with the possibility of extension. This position is full-time (35 hours/week); however, consideration will be given to candidates seeking reduced hours where a strong fit is identified.
The
Foundations Program
is a supportive transitional program designed to help young adults, ages 18 to 26, successfully move from student life into greater independence and engagement within themselves and their community. Grounded in the values of inclusion, collaboration, and personal growth, the program empowers participants to build a life of meaning, belonging, and choice. Together, we focus on developing essential life skills which include employment and volunteer readiness, dailyliving and personal management, social and independent skills, and active involvement in community recreation and leisure opportunities. Through hands-on learning, groups, workshops, community partnerships, and individualized support, participants strengthen their confidence, self-advocacy, and ability to navigate adult life.
Job Summary:
The Foundations Mentor is accountable for establishing a supportive working relationship with adults in the Foundations program. The Mentor identifies client's strengths and targets personal goals. They are adaptable to change and pivot support strategies as needed. They utilize a person-centred approach and lead with creativity when problem solving. They work well independently and in groups. They are a self starter and hold a deep respect for diversity, individuality, and the dignity of each participant. Commitment to person-centered planning that supports autonomy, choice, and inclusion. Exceptional interpersonal and communication skills while engaging with participants, families, staff, and community partners. The ability to build trusting relationships and foster a positive, team-oriented culture. Skilled at conflict resolution and maintaining professionalism under pressure. Demonstrated ability to guide, mentor, and inspire. Encourages skill development, self-advocacy, and confidence in others. Nurtures a supportive environment that balances structure with compassion.
Qualifications:
- Developmental Services Worker (DSW) diploma preferred or an acceptable equivalent with strong background in Developmental Services. Consideration will be given to those with a complimentary degree or related diploma from a recognized institution.
- The successful candidate must possess excellent written and oral communication skills, organizational skills, computer skills, maturity, tact, discretion, and the ability to analyze client and organizational needs.
- The successful candidate must possess the ability to independently support individuals one-to-one, facilitate group activities, have good interpersonal skills, and must have the ability to develop positive working relationships with individuals supported, families, colleagues, and community partners.

- Valid driver's license with $2 million liability automobile insurance
