What We are Looking For:
We are currently recruiting for a Registered Practical Nurse to join our Diabetes Education Team for a part-time position. This role is responsible for providing comprehensive foot assessments, routine treatments, health education, and prevention services to support clients- particularly those living with diabetes, in maintaining optimal foot health and preventing complications.
A successful candidate in this role would have
Registered Practical Nurse with the College of Nurses of Ontario in good standing
Completion of an accredited Advanced or Diabetic Foot Care certificate program.
Experience providing foot care services in community or diabetes care settings preferred.
Demonstrated ability to work independently and collaboratively within an interdisciplinary team.
Proficiency in electronic health records, preferably Practice Solutions.
Strong communication, assessment, and health education skills.
In this role you will be expected to complete the following tasks:
Perform comprehensive and basic foot assessments, delivering routine foot care services within the Registered Practical Nurse (RPN) scope of practice and in accordance with College of Nurses of Ontario (CNO) standards.
Provide safe and compassionate foot care, including nail trimming, skin care, and management of corns, calluses, and ingrown nails.
Observe, identify, and document early signs of infection, ulceration, or other complications, initiating timely referrals to chiropodists, podiatrists, or other specialists as required.
Maintain strict adherence to infection prevention and control practices, including proper instrument cleaning and sterilization.
Accurately document assessments, interventions, and client outcomes in accordance with organizational policies and confidentiality requirements.
Educate clients and families on foot health, hygiene, appropriate footwear, and self-monitoring, emphasizing diabetes-related foot care.
Promote client self-management and provide individualized teaching to support prevention of foot complications.
Work collaboratively with interdisciplinary team members including diabetes educators, nurses, and primary care providers to ensure coordinated, holistic client care.
Contribute to team meetings and care planning while serving as a resource on foot care practices and referral pathways within the Diabetes Education Program.
Perform additional related duties as assigned to support program goals and client well-being.

The successful candidate would be eligible for the following benefits:
Salary of $26.33-$31.26 Hourly
Paid 30-minute lunch for shifts exceeding 5 hours.
5 Sick/Personal Days per year (pro-rated to hours worked)
Enrolment in Healthcare of Ontario Pension Plan (HOOPP)
